门冬氨酸鸟氨酸治疗肝硬化并发肝性脑病临床研究  被引量:9

摘  要:目的观察门冬氨酸鸟氨酸(瑞甘)治疗肝硬化并发肝性脑病的临床疗效。方法113例肝硬化并发肝性脑病病人随机分成瑞甘组40例、雅博司组38例、精氨酸组35例。三组病人均给予治疗原发病、去除诱因、低蛋白饮食、补充支链氨基酸、乳果糖及护肝等治疗,在此基础上瑞甘组加用瑞甘10~20g/d;雅博司组加用雅博司20g/d;精氨酸组加用精氨酸钠20g/d,三种药物均用7d。监测治疗前后检测血氨、AST、TBIL等指标及神志变化。结果门冬氨酸鸟氨酸对降低血氨、AST、TBIL和神志改善等明显优于精氨酸(P〈0.05),但门冬氨酸乌氨酸组和雅博司组相比无显著差异(P〉0.05)。结论瑞甘治疗肝硬化并肝性脑病疗效确切,效果与雅博司相当,且其不良反应发生率低、规格合理,有临床推广价值。Aims: to observe the effect of L-omithine-L-aspartate in the treatment of liver cirrhosis complicated with hepatic encephalopathy. Methods 113 cases of liver cirrhosis complicated with hepatic encephalopathy patient are divided into Rigan group 40 cases, the hepa-merz group 38 cases, the arginine group 35 cases stochastically. Three group of patients are given treating the primary disease, elimination of precipitating factors, the low protein diet, supplement of branched chain amino acid, Lactulose and liver protection treatment and so on, based on these treatment Rigan group plus Rigan 10-20g/d; hepa-merz group plus hepa-merz 20g/d; Arginine group with arginine sodium 20g/d, for 7d. Before and after the treatment examine the blood ammonia, AST, TBIL and the state of mind and so on. Results L-ornithine-L-aspartate reduces the blood ammonia, AST, TBIL and improve the state of mind, it surpasses the arginine obviously ( P〈0.05 ) . but there is no significance difference between Rigan group and hepa-merz group ( P〉0.05 ) . Conclusions L-omithine-L-aspartate has definite effect in the treatment of liver cirrhosis complicated with hepatic encephalopathy, almost same as hepa-merz .and the liver brain illness curative effect is accurate, its incidence of side effect is low, and specification is reasonable, it has the value for clinical use.
