癌旁胃粘膜肠化与胃癌PCNA、癌基因蛋白产物同步检测对比分析

摘　　要：对７０例胃癌伴癌旁胃粘膜肠化石蜡标本连续切片组织同步进行ＰＡＳ—高铁二胺—爱先蓝复合粘液染色及增殖细胞抗原（ＰＣＮＡ），Ｐ５３、Ｐ２１、ｃ－ｅｒｂＢ－２三种癌基因蛋白产物检测，结果显示：癌旁胃粘膜肠化以结肠型化生为主（８７．１％），包括不完全性结肠型化生和完全性结肠型化生。２２．９％的胃癌组织中含有与不完全性结肠型化生相同的粘液，５４．３％含有与完全性结肠型化生相同的粘液，表明胃粘膜结肠型化生与胃癌有关；小肠型化生，结肠型化生，胃癌ＰＣ－ＮＡ指数逐级升高，表明胃粘膜肠化与胃癌均有不同程度的异常增生；正常胃粘膜Ｐ５３、Ｐ２１、ｃ－ｅｒｂＢ－２不表达，小肠型化生表达阳性或弱阳性，无复合表达，不完全性结肠型化生、完全性结肠型化生、胃癌表达率、过度表达率、复合表达率逐级递增，提示胃粘膜结肠型化生作为一种癌前病变或早期胃癌的一个信号，应引起临床医生高度重视。The expression of protein products of oncogene P53,P21 and c-erbB-2 were studied by means of PAS-HID-Alcian blue staining method in serial tissue sections of 70 cases of gastric carcinoma with intestinal metaplasia in gastric mucosa.The results showed that 87.1% was the colon metaplasia besides carcinoma of stomach,include complete and incomlete colon metaplsia 22.9% of tissue was in carcinoma of stomach and incomplete colon metaplsia.It showed that there were relative association between colon melaplisa and gastric carcinoma.There were different abnormal proliferaton in intestinal metaplasia of gastric mucosa and carcinoma with the gradually increasing of small intestinal and colon metaplasia and PCNA of gastric carcinoma.It did not express the P53,P21,c-erbB-2 in normal gastric mucosa.The small intestinal metaplasia expressed positive or weak positive,but no compound expression.The excess and compound expression rate increased gradually in incomplete and complete colon metaplasia and gastric carcinoma.It showed that the colon metaplasia of gastric mucosa was possiblly a sign of earlier gastric carcinoma and that multigene activated played the promotion of devolping the intestinal metaplsia of gastric mucosa into gastric carcinoma.
