多发进展期胃癌第三站淋巴结转移的相关因素分析

摘　　要：目的:探讨多发进展期胃癌第3站淋巴结转移规律,指导手术清除范围,以期提高根治效果。方法:回顾性分析11例发生第3站淋巴结转移多发进展期胃癌患者的临床病理资料,按照主癌大小、主副癌组织学、大体分型、主副癌所占分区、淋巴管癌栓进行统计学分析。诊断依赖于病理组织学检查。结果:主癌大小和主、副癌所占分区对第3站淋巴结转移差异有统计学意义,P=0.002。其他因素如主、副癌组织学分型、大体分型、淋巴管癌栓差异无统计学意义,P>0.05。主癌≥5 cm组7例第3站淋巴结转移中No.12转移3例,No.13转移3例,No.14转移1例。主、副癌位于不同区域组16例第3站淋巴结转移中No.11转移4例,No.12转移6例,No.13转移4例,No.14转移1例,No.15转移1例。结论:主癌≥5 cm和主、副癌位于不同区域是影响多发进展期胃癌第3站淋巴结转移的独立因素,对于主癌≥5 cm、主副癌位于不同区域者宜行D2+根治术式。OBJECTIVE： To explore the correlation factors of No.3 metastasis of multiple advanced gastric carcinoma in order to guide the extent of radical excision and improve its effect.METHODS：The data from 11 patients with multiple advanced gastric carcinoma and surgically treated were collected,and the association between the clinicopathological factors and the presence of No.3 metastasis was retrospectively analyzed by univariate analysis.RESULTS：The diameter of the main carcinoma and area occupied by the carcinoma were significantly associated with a higher rate of No.3 metastasis,P=0.002.However,the macroscopic type,type of the tissue and lymphatic vessel involvement were not associated with No.3 metastasis,P〉0.05.In the 7 cases with No.3 metastasis in the main carcinoma ≥5 cm group,there were 3 cases with No.12 metastasis,3 cases with No.13 metastasis and 1 case with No.14 metastasis.In the 16 cases with No.3 metastasis in different areas occupied by the carcinoma,there 4 cases with No.11 metastasis,6 cases with No.12 metastasis,4 cases with No.13 metastasis,1 case with No.14 metastasis and 1 case with No.15 metastasis.CONCLUSIONS： No.3 metastasis of multiple advanced gastric carcinoma are associated with the main carcinoma ≥5 cm and different areas occupied by the carcinoma.The D2 ＋ operation should be carried out in No.3 metastasis of diameter multiple advanced gastric carcinoma,especially for the main carcinoma ≥5 cm and different areas occupied by the carcinoma.
