摘　　要：目的：总结免疫组织化学技术在前列腺癌（PCa）早期诊断中的应用。方法：应用Medline、EMCC及CNKI期刊全文数据库检索系统,以“α-甲基酰基辅酶A消旋酶、前列腺肿瘤、诊断”等为关键词,检索2000-01-2008-05相关文献。纳入标准：1）前列腺基底细胞标志。2）AM-ACR在前列腺组织中的表达。3）AMACR的临床应用。4）AMACR与34βE12、p63的联合应用。粗选有114篇AMACR与PCa诊断方面的文章,根据纳入标准,精选58篇文献,最后纳入分析24篇文献。结果：在前列腺针刺活检标本中,AMACR检测PCa的敏感性为80%～100%,特异性为70%～100%,具有较高的敏感性及特异性。CK34βE12、p63是前列腺基底细胞标志,在PCa中呈阴性表达。结论：AMACR联合CK34βE12、p63能够提高PCa的早期诊断率,在PCa的发生、发展、诊断和治疗方面具有重要的应用价值。OBJECTIVE：To summarize the diagnostic utility of immunohistochemistry in prostate cancer whose diagnosis is morphologically difficult.METHODS：The pertinent literature from January,2000 to May,2008 in Medline,EMCC and CNKI full-text data base was searched,and the of index were AMACR,prostate cancer and diagnosis.The criteria of selection： 1）the basal cell markers of prostatic gland,2）AMACR expression in prostatic tissue,3）clinical application of AMACR,4）The application of AMACR combined with CK34βE12 and p63 in early diagnosis of prostate carcinoma.There were 114 papers of AMACR associated with the diagnosis of prostate cancer and 58 papers according to the selection criterian,and 24 papers were selected to be analyzed.RESULTS：AMACR as a prostate-specific marker have good sensitivity and specifity.By the moderate or strong staining intensity as positive,the evaluation of AMACR protein expression in prostate needle biopsy specimens demonstrate 80%-100% sensitivity and 70%-100% specificity for detecting prostate cancer.CK34βE12 and p63 are the basal cell markers of prostatic gland,but their expressions are regative in prostate cancer.CONCLUSION：The application of AMACR combined with CK34βE12 and p63 may be of greater benefit in early diagnosis of prostate adenocarcinoma.
