术前区域动脉热化疗对进展期胃癌病理学的影响

摘　　要：目的:评价进展期胃癌行术前区域性动脉热化疗对胃癌组织的病理学的影响。方法:应用原位末端标记法(TUNEL)及免疫组化技术对107例进展期胃癌组织标本进行凋亡和细胞增殖的检测;107例进展期胃癌分3组,A组38例,采用单纯动脉灌注化疗,B组42例,采用动脉热灌注化疗,C组27例,单纯外科手术作对照。结果:A组进展期胃癌的细胞凋亡率为10.6%±4.5%,B组的细胞凋亡率15.9%±2.8%,C组的细胞凋亡率为4.4%±1.5%,两两组间差异均有统计学意义(P<0.05)。PCNA的阳性率A组为57.9%(22/38),B组为33.3%(14/42),C组为85.2%(23/27)。结论:术前动脉灌注化疗及热化疗均可促进进展期胃癌组织的细胞凋亡并抑制其增殖,可作为进展期胃癌术前辅助化疗重要方法之一。Objective：To evaluate the effect of histopathology response of local preoperative thermal intra-arterial chemotherapy on advanced gastric carcinoma. Methods： 107 cases with advanced gastric cancer were determined whether they experienced apoptosis and proliferature by TUNEL and immunochemical technology. 107 cases were derided into 3 groups, i.e, group A： 38 cases were only treated with intra-artery infusion chemotherapy, group B： 42 cases underwent thermal intra-artery infusion chemotherapy, group C： 27 cases were treated with surgery used as controls.Results： The rates of apoptosis of gastric carcinoma of group A, group B and group C were （ 10.6 ± 4.5）% ,（15.9±2.8）% and （4.4± 1.5）%, respectively. The difference between two groups had statistic significance （ P 〈0.05）. The positive rates of PCNA for group A, group B and group C were 57.9 % （23/38）, 33.3 % （ 14/42 ） and 85.2 % （23/27）, respectively. Conclusion： Preoperative intraartery infusion chemotherapy and thermal intraartery infusion chemotherapy can promote apopotosis of advanced gastric carcinoma and restrain their proferation, which may serve as an important adjuvant treatment for advanced gastric cancer .
