胃黏膜不同病变组织中突变型P53蛋白表达的研究

摘　　要：目的通过对胃癌高危人群胃黏膜不同病变组织中突变型P53蛋白的检测,探讨在胃癌高发区胃黏膜不同病变组织中突变型P53蛋白表达的意义。方法采用免疫组化S-P法检测了本研究的胃黏膜标本178例来自胃癌高危人群普查和63例胃癌组织临床活检标本突变型P53蛋白的表达情况。结果在178例胃黏膜组织中,突变型P53蛋白在CSG、CAG、伴IM、伴DYS和GC中表达率分别为4.90%、25.00%、40.00%、55.55%和60.31%。在CSG中的表达率最低,胃癌中表达率最高,随着病变的进展阳性表达率呈递增趋势,其中GC与CAG、CSG之间差异有显著性意义(P<0.005)。63例胃癌组织中,突变型P53蛋白在20例高分化腺癌中阳性表达率为35.00%,20例中分化腺癌中阳性表达率为60.00%,23例低分化腺癌中的阳性表达率为86.95%。低分化腺癌中的阳性表达率与高分化腺癌中的阳性表达率相比有显著性(χ2=12.36,P<0.005),低分化腺癌中的阳性表达率与中分化腺癌中的阳性表达率相比有统计学意义(χ2=4.08,P<0.05)结论突变型P53蛋白的阳性表达率随着病变的发展而增高,提示P53基因的突变与胃癌的发生有关,可能是安塞县胃癌高发的一个重要因素。Objective To investigate the relationship the expression of mutable P53 protein in the different pathological tissue of gastric mucosa. Methods The immanohistochemical staining technique （S -P method） was used to detect the expressions of Runx3 178 specimen in the population at gastric cancer risk and 63 specimen gastric cancer. Results ①The protein expression rate of mutable p53 in CSG, CAG, IM, DYS, GC are 4.90%, 25.00% ,40.00% , 55.55% and 60.31% respectively. The protein expression of muta： ble p53 is the lowest in CSG, and with the lesion＇ s worsening, the expression rate is increasing in that of gastric cancer the highest, up to 60.31%. The differences among CAG, CSG and GC had obvious significance（P 〈0. 005 ）. ② In 63 cases of gastric cancer, the expression rate of mutable P53 protein in well differentiated, moderately differentiated and poorly differentiated carcinoma are 35.00%, 60.00%, 86.95% respectively, with the lesion＇ s worsening, the expression rate is increasing in poorly differentiated carcinoma. Conclusion The protein expression of mutable P53 protein with the lesion＇ s worsening, it is cuing the mutable p53 has much to do with gastric cancer, and it is possible an important factor to the population at gastric cancer risk.
