rhEGF联合单径路隐形支架修复泪小管及恢复泪道功能的研究

摘　　要：目的探讨重组人表皮生长因子（recombinant human epidermal growthfactar，rhEGF）滴眼液联合单径路隐形支架植入治疗泪小管阻塞的效果。方法泪小管阻塞200例（256只眼），在泪道植入自制聚乙烯支架后，根据泪小管阻塞的3个划分部位，符合条件的按首诊次序交替分组，分为常规治疗的对照组100例（126只眼）及在常规治疗基础上增加rhEGF滴眼液滴眼的实验组100例（130只眼）。2月后去支架，随访18月。并在6、12、18月的3个时间段两组以治愈率进行对比。在18月时还对两组阻塞部位与疗效进行对比分析。结果所有患者术眼均成功置入支架。经主诉和泪道冲洗确定疗效，实验组和对照组治愈率：在6月时分别为88．46％和84．92％，差异无统计学意义（Χ^2=0．697，P〉0．05）；12月时分别为80．77％和69．84％，差异有统计学意义（Χ^2=4．118，p〈0．05）；18月时分别为80．00％和60．32％差异有显著统计学意义（Χ^2=11．825，P〈0．01）。18月时，7～8mm阻塞部位实验组和对照组治愈率分别是86．59％和72．50％，差异有统计学差异（Χ^2=4．953，P〈0．05）；5-6mm阻塞部位实验组和对照组治愈率分别是77．50％和47．37％，差异有显著统计学意义（Χ^2=7．575，P〈0．01）。结论植入自制支架后使用rhEGF滴眼液，能显著提高泪小管阻塞的远期治愈率，整个过程简单易行，容易推广。Objective To study the effect of recombinant human epidermal growth factor,rhEGF eye drop combined with single pathway anastomosis stent placement in treating obstruction of lacrimal canalicalus. Methods 200 patients （256 eyes）with obstruction of lacrimal canlicalus were treated with the placement of self-made polyethylene stents.Patients were alternatively grouped into control group （100 patients 126eyes treated rountinely）and experimental group （ 100 patients 130eyes treated with rhEGF eye dropping on the basis of rountine treatment）according to first diagnosis and 3 classified area of lacrimal canaliclus.2 month later, the stents were removed, and following survey were carried on in the 18 months afterward.And the cure rates of the 2 groups were compared in 3 different time period：6 months, 12 months and 18 months.At the end of 18 months we also compared and analyzed the treating effects toward obstruction positions of the 2 groups. Results anastomosis stents were all successfully implanted into the patients.Effect of treatment were determined by chief complaints and flushing lacrimal ducts, the cure rates of experimental and control groups are given as following： 88.46% and 84.92% respectively in 6 months, showing no statistical difference （Χ^2=0. 697, P 〉0.05 ） ; 80.77% and 69.84% respectively in 12 months, showing statistical difference （Χ^2=4.118, P 〈0. 05 ）;80.00% and 60.32% respectively in 18 months,showing significant statistical difference（Χ^2=11.825 ,P 〈0. 01 ）.In 18 months, the cure rates are 86.59% and 72.50% for 7-8mm obstruction position of experimental and control groups respectively,manifesting significant statistical difference （Χ^2=4.953,P 〈0.05）;77.50% and 47.37% for 5-6mm obstruction position of experimental and control groups respectively,showing statistical difference （Χ^2=7.575,P 〈0.01 ）.Conclusions rhEGF eye drop treatment after implanting self-made stents can significantly improve the cure rate of obstruction of obstruction.The whole pr
