眼附属器淋巴瘤的影像学表现及其临床意义

摘　　要：目的了解眼附属器淋巴瘤的CT和MRI表现。方法回顾性分析经手术或活检病理证实的50例眼附属器淋巴瘤的影像学表现，其中31例行CT扫描，8例MRI扫描，11例行CT和MRI扫描。结果侵犯眼睑或结膜18例，泪腺6例，肌锥内外11例，肌锥外8例，肌锥内2例，眼外肌受累16例，视神经受累13例。7例肿瘤呈铸造样包裹整个眼球。结论眼附属器淋巴瘤的影像学表现有一定特征性，MRI脂肪抑制技术有助于了解肿瘤范围。Objective To study the CT and MRI features of ocular adnexal lymphoma and to improve the knowledge for the clinical diagnosis of the disease.Methods CT and MRI findings of 50 cases （51 eyes） with pathologically proved adnexal ocular lymphoma were respectively analyzed,bilateral involvement were detected in 1 cases.31 cases underwent CT scanning, 8 cases underwent MR scanning, both CT and MRI were performed in 11 eases.Results Multiple locations of involvement were frequently revealed.Eyelids or conjunctivae invasion were assessed in 18 cases,lacrimal gland invasion in 6 cases,intra muscuclar cone invasion in 2 cases, extra cone invasion in 8 cases, simultaneous intra and extra cone invasion in 11 cases.24 lesions showed diffuse infiltration,nine lesions were nodular alike, 17 lesions showed trabs shape.The lesions which were located within the orbits of 7 showed molding growth pattern along the ocular structures, such as global wall,involving extra ocular muscles and optic nerve.Of the 42 patients had plain CT,homogeneous attenuation were showed in 40 cases, while 2 lesions were heterogeneous.Of the 31 patients had enhanced CT, 23 patients showed homogeneous enhancement while 6 patients showed heterogeneous enhancement.Cystic necrosis（n = 2 ）were founded.In the 19 patients having MRI, 16 cases showed homogeneous signal intensity on T 1WI, while 3 cases showed mild signal intensity. 18 cases showed homogeneous signal intensity on T2WI MR images,while 1 case showed slightly high signal intensity.In the 13 patients having enhanced MRI, 10 cases showed homogeneous enhancement after contrast administ ration,while three cases showed heterogeneous enhancement.Conclusion Ocular adnexal lymphoma showed characteristic imaging manifestations,the contour of the tumor and the range of invasion could be clearly delineated on CT and MRI with enhancement technique.
