肾血管平滑肌脂肪瘤超声造影表现的研究

摘　　要：目的总结和探讨肾平滑肌脂肪瘤的超声造影特征。方法22例肾血管平滑肌脂肪瘤共26个病灶接受了常规超声和超声造影检查。分析病灶的常规超声及超声造影表现特征。超声造影使用造影剂声诺维和对比脉冲序列成像技术。结果二维超声显示低、等、高回声的病灶,分别为0(0/26)、7.7%(2/26)和92.3%(24/26)。病灶彩色血流信号显示率为11.5%(3/26),动脉峰值流速(33.6±5.5)cm/s(29.2～39.7cm/s),阻力指数0.58±0.04(0.55～0.63)。超声造影显示皮质期20个(76.9%)病灶表现为等或高增强,6个(23.1%)为低增强。实质期及晚期所有病灶增强均未见明显减退。84.6%(22/26)的病灶与肾实质同时增强,80.8%(21/26)的病灶表现为周边先增强,然后迅速向心性全瘤增强特点。结论与肾皮质同步的均匀增强、病灶三期增强水平一致不减退、向心性增强模式及无包膜是肾血管平滑肌脂肪瘤的重要表现,将有助于诊断及鉴别诊断。Objective To conclude the characterization of renal cellular carcinoma with contrast-enhanced Ultrasound.Methods Twenty-two patients（twenty-six nodules） with renal angiomyolipoma,which was confirmed by operation and followup.underwent conventional ultrasound and contrast-enhanced ultrasound.microbubble contrast agents and low mechanical index continuous imaging technique of contrast pulse sequence were used.the conventional Ultrasonographic characterization and the contrast enhanced pattern of lesions was analysed.Results Twenty（76.9%）lesions were hypervascular,and six （ 23.1%）were hypovascular in cortical phase,All lesions were not wash-out markedly in late phase.Twenty-one（80.8%） lesions were periphery-to-center enhancement.Conclusions The critical contrast-enhanced ultrasonic manifestations of renal angiomyolipoma are hypervascular and periphery-to-center enhancement in cortical phase,and not wash-out in late phase.And it may be helpful for differential diagnosis.
