彩色多普勒超声在腮腺良恶性肿瘤鉴别中的应用

摘　　要：目的:探讨彩色多普勒血流显像(CDFI)、彩色多普勒能量图(CDE)在腮腺良恶性肿瘤鉴别中的作用。方法:用二维声像图(2DUS)、彩色多普勒血流显像、彩色多普勒能量图等超声方法检查腮腺肿瘤90例,按肿块的2DUS特征和血流特点判定为良性和恶性,检查结果与手术后病理对照,将同一肿块的CDFI与血流显示情况及其血流动力学指标进行对比分析。结果:CDFI显示恶性肿瘤的血流信号检出率高,良、恶性肿物的血流信号检出率分别为34.62%(18/52)、81.58%(31/38)。CDE显示恶性肿瘤的血流信号更丰富,两者有显著性差异。结论:彩色多普勒超声在腮腺良恶性肿瘤诊断中具有重要的价值。Objective： To investigate the clinical value of color Doppler flow image （CDFI） in the differential diagnosis of parotid gland malignant tumors from benign ones. Methods： Ninety cases of parotid glands tumors were detected by 2D ultrasound, CDFI and color Doppler energy （CDE）. The results of ultrasound examination （including 2D ultrasound, CDFI and CDE） and postopera- tive biopsy were compared. Results： CDFI demonstrated that detective rate of the malignant tumor was high. Blood flow signals could be detected by CDFI in 34.62% （18/52） of benign tumor, while the detective rate of malignant tumor was 81.58% （31/38）. CDE showed richer blood flow signals of malignant tumors under the same conditions and there were significant differences between CDFI and CDE. Conclusion; Color Doppler ultrasound is valuable in the differential diagnosis of the parotid gland tumors.
