脱细胞异体真皮基质在唇裂术后继发唇鼻畸形修复中的初步应用

摘　　要：目的:应用脱细胞异体真皮基质(AADM)矫治唇裂术后继发畸形中的患侧鼻底塌陷、组织量不足,上唇及红唇组织量不足。方法:在矫治唇裂术后继发畸形中用AADM,按所需增加的组织量修剪、塑形为相应形态后,植入鼻底塌陷处,鼻底复合软组织深层、隆鼻术层及口轮匝肌深层。结果:本组病例共36例,均为鼻底塌陷填充,其中17例同时作AADM隆鼻及鼻尖塑形,12例同时作AADM丰唇。除7例鼻底塌陷矫正不足,3例丰唇双侧不完全对称外,效果均满意。结论:AADM作为同种异体软组织填充材料,用于唇裂术后继发畸形中组织量不足的填充,可根据患部所需随意剪裁塑形,既避免了外源合成材料填充的缺陷,又解决了切取自体组织二次损伤及美容瘢痕残留问题。Objective To repair the secondery labionasal deformities after cleft lip operation with AADM. Metheds We shaped and chiped AADM according to needed tissue, and implanted AADM in dent of basis nasi,deep of tisses,the layer of operation,and the deep of orbicular muscle of mouth. Results 36 patients were all filled for basis nasi deformity. 17 cases have been augmentation rhinoplasty by AADM and shaped the apex nasi at one time, 12 cases were fulled lips at one time. 7 cases whose basis nasi were dented were rectified incompletely, 3 cases that have been fulled lips were imperfection. Others were all satisfactory. Conclusions It is a good methed that repair labionasal deformities after cleft lip repair with AADM. tt can randomly cut out the shape, avoid the deficiencies of other metheds, and solve the problems of secondary damnification after autograft.
