微创术配合神经因子腔内用药治疗超早期高血压壳核出血分析

摘  要:目的研究应用微创术配合神经生长因子(NGF)腔内用药治疗超早期高血压壳核出血临床疗效。方法治疗组及对照组均行超早期软通道技术-微创颅内血肿液化引流术,破入脑室者同时行侧脑室额角穿刺脑脊液外引流术。治疗组术后在常规治疗的基础上给予NGF 2ml/d血肿腔内注射,4d后改为NGF 2ml/d,肌内注射。对照组在常规治疗的基础上给予胞二磷胆碱治疗。2组疗程均为14d。采用日常生活活动量表(ADL)评分标准对治疗3个月后进行疗效评定。结果治疗组86例病人除1例出血破入脑室死于多器官功能衰竭外,余85例均存活,ADL133例,ADL227例,ADL322例,ADL43例,优良率(ADL1+2+3)95.4%,致残率(ADL4+5)4.6%。与对照组比较有显著性差异(P<0.01),明显提高了病人的生存质量。结论NGF能提高微创术后高血压壳核出血的临床疗效,降低致残率,明显提高病人的生活质量,血肿腔内用药效果更好。Objective To research the effect of micro creates technique cooperated with nerve growth factor (NGF) cavity internal medicine treating the ultra early hypertension shell nucleus to bleed. Methods Treatment group and control group were given the line of ultra early soft channel technology - micro creates in the skull the haematoma liquefication drainage. Patients broken enters the ventricles of brain were also given the fashionable ventriculi laterals enciphali frontal eminence puncture cerebrospinal fluid outside the drainage. Treatment group was given NGF of 2ml/d in the conventional treatment foundation, haematoma cavity to inject every day, after 4d every day NGF 2ml by intramuscular injection. Control group was given the butcher in the conventional treatment foundation two phosphorus choline treatment. Treatment courses of two groups were 14 days. Activity of daily life (ADL) was used to evaluating the curative effect. Results Besides 1 example broken enter the ventricles of brain die of multi-organ function failure, 85 examples survived, ADL1 of 33 examples, ADL2 of 27 examples, ADL3 of 22 examples, ADL4 of 3 examples, fine rate (ADL1+2+3) was 95.4%, crippling rate (ADL4+5) was 4. 6%. There was a significance difference compared with control group (P〈0. 01). Conclusion NGF can enhance the curative effect of the hypertension shell nucleus hemorrhage affter micro create opereation, and reduces crippling rate, improves the quality of life obviously. The haematoma cavity internal medicine effect is better.
