摘  要:目的探讨多排螺旋CT三维测量技术在齿科正畸中的应用。方法对30例齿科正畸病例进行多排螺旋CT扫描和X线头影摄片,运用容积重建(VR)和多平面重建(MPR)三维成像方法进行重建,对比测量颌骨及牙齿的角度、线距等指标。结果X线头影测量组在颌骨前后向关系的指标SNA、SNB、FH/NP的测量值均小于三维CT测量组,两者比较有显著性差异(P均<0.01),ANB测量值两者比较无显著性差异(P>0.05)。在颌骨垂直向关系的指标MP/FH、Pg-NB的测量值无显著性差异(P均>0.05)。牙齿角度及线距指标U1-NA、U1/NA、L1-NB、L1/NB的测量值均大于三维CT测量组,2组比较有显著性差异(P均<0.01);U1-NA、U1/NA、L1-NB测量值2组比较无显著性差异(P均>0.05)。结论多排螺旋CT三维测量技术在齿科正畸中应用具有可行性,较X线头影测量具有优势;但测量方法及诊断标准尚需制订。Objective It is to approach the applied value of 3D imaging technique of Multi-detector CT in dental orthopaedics. Methods 30 cases of dental orthopaedics were underwent Multi-detector CT and X-ray photograph to compare the measured value of the angle, distance of jaw bones and teeth by volume reconstruction (VR) and multi-plane reconstruction (MPR). Results The measure values of jaw bones sagittalis indexes including SNA, SNB and FH/NP in group X-ray cephalometric were all less than in group CT 3D measure and there were significant differences between them (P 〈 0.01 ). As for the measure values of ANB, there was no significant differences between them (P 〉 0.01). There was no significant differences between them (P 〉0.01 ) as to the measure values of jaw bones vertical indices including MP/FH and Pg-NB. The measure values of teeth angles and distances indices including U1 - NA, U1/NA, L1 - NB and L1/NB were all greater than CT 3D measure value. As for the measure values of L1 - NB and L1/NB, there were significant differences between them (P〈 0.01 ). There were no significant differences between them as to the measure values of U1 - NA, U1/NA and L1 - NB (P 〉 0.01 ). Conclusion Although 3D imaging technique of Multi-detector CT in dental orthopaedics is feasible and is superior over X-ray cephalometric, it need institute the method of measurement and diagnostic criteria.
