结膜松弛症球结膜杯状细胞研究

摘　　要：目的观察结膜松弛症球结膜杯状细胞的组织病理学及超微结构改变。方法对10例松弛球结膜组织和10例正常对照组球结膜对应区组织,采用PAS和AB染色,对照观察组织病理及超微结构改变。结果结膜松弛症球结膜上皮中的杯状细胞密度为(2.75±2.68)/100个上皮细胞,对照组为(4.64±3.25)/100个上皮细胞,2组间差异无统计学意义(t=1.42,P=0.17)。随着结膜松弛症程度加重,球结膜杯状细胞密度降低,Ⅰ级(3.90±2.40)/100个上皮细胞,Ⅱ级(2.96±0.72)/100个上皮细胞,Ⅲ级(2.05±0.86)/100个上皮细胞,Ⅳ级(1.33±0.96)/100个上皮细胞。结膜松弛症球结膜上皮细胞层有鳞状化改变,细胞层数较厚,为4～15层,固有层很薄,电镜观察结膜松弛症杯状细胞胞质内大量浆液性分泌颗粒,线粒体少。而正常对照组球结膜上皮层较薄,为4～5层,电镜观察杯状细胞胞质中有黏液小滴,并有丰富的线粒体,杯状细胞顶端有微绒毛。结论结膜松弛症中球结膜杯状细胞数量减少,形态及超微结构有异常。Objective To observe the histopathologic ultrastructural changes of goblet cell in conjunctivochalasis by electron microscope. Methods Conjunctival tissue were collected from 10 patients with conjunctivochalasis and from 10 normal volunteers. With PAS and AB staining, histopathologic changes and ultrastructural changes of conjunctival tissue were observed in patients with conjunctivochalasis and in normal volunteers. Results The proportion of goblet cell in conjunctivochalasis was（2.75 ±2.68）/100 epithelial cells,while that of normal volunteers was（4.64 ± 3.25 ）/100 epithelial cells,there was no significant difference between two groups （ t = 1.42,P =0.17 ）. The proportion of goblet cell was （ 3.90 ± 2.40 ）/100 epithelial cells in Ⅰ degree conjunctivochalasis, （ 2.96 ± 0. 72）/100 epithelial cells in Ⅱ degree conjunctivochalasis, （2.05 ± 0.86）/100 epithelial cells in Ⅲ degree conjunctivochalasis, （ 1.33 ± 0.96 ）/100 epithelial cells in Ⅳ degree conjunctivochalasis. With the severity of conjunctivochalasis increasing, the proportion of goblet cell was reducing. The bulbar conjunctival epithelium had squamatization changes. The layer of the cells was thick,about 4 to 15 layers. Lots of serosity secretory granules accumulated and little mitochondrions was seen under the electron microscope. The layer of the cells was thin, about 4 to 5 layers. There were mucus droplets and lots of mitochondrions in the cytoplasm of the normal controlled goblet cells. Microvilli was found at the top of the goblet cells. Conclusion In conjunctivochalasis, the number of the goblet cells decrease, the shape and ultrastructure are abnormal.
