摘  要:目的观察新辅助化疗(NACT)的临床疗效以及NACT前后宫颈癌组织中中期因子(MK)、微血管密度(MVD)的表达变化。方法对35例宫颈鳞癌患者用顺铂+平阳霉素行NACT,并采用免疫组化SP法检测癌组织NACT前后MK、CD34的表达。结果35例宫颈癌患者接受NACT后,临床完全缓解(CR)4例(11.43%),部分缓解(PR)25例(71.43%),总有效率(CR+PR)82.86%;NACT后MK表达(31.4%)较NACT前(74.3%)下降(P<0.05);MVD水平在NACT后(16.57±7.60)较NACT前(24.65±9.17)亦有明显下降(P<0.05);临床有效组MK表达在NACT后(31.0%)较NACT前(75.9%)显著下降(P<0.05),而临床无效组MK表达在NACT后(33.3%)较NACT前(66.7%)表达差异无统计学意义(P>0.05);临床有效组MVD计数在NACT后(16.27±7.49)较NACT前(24.17±8.55)显著下降(P<0.05),临床无效组MVD计数在NACT后(21.02±8.70)较NACT前(26.98±12.42)亦有明显下降(P<0.05)。结论NACT对宫颈癌有效;NACT后MK、MVD表达下降。NACT发挥效应的机制之一可能为抑制肿瘤血管生成。检测MK在NACT前后的表达变化可协助预测NACT的疗效。Objective To investigate the clinical effect of neoadjuvant chemotherapy (NACT) and its influente on the expressions of MK and MVD in cervical cancer cells. Methods The expressions of MK, CD34 were determined by immunohistochemistry in 35 cases of cervical cancer before and after NACT. Results Among the 35 cases treated with NACT, 4 (11.43%) were complete remission (CR), 25 (71.43%) partial remission (PR). The total effective rate of NACT (CR+PR) was 82.86%. The expressions of MK and MVD decreased dramatically after NACT. In cases who reacted positively to NACT, the expression of MK and the level of MVD decreased significantly after the treatment (P〈0. 05). While in cases who reacted poorly to NACT no such changes were observed in the expression of MK (P〉0. 05) ; but the level of MVD also decreased in those cases. Conclusion NACT is an effective treatment for eervieal cancer. The expressions of MK and MVD decreased after NACT treatment. It implied that one of the mechanisms of NACT may be the suppression of angiogenesis, and the expression of MK may serve as an indicator for predicting the therapeutic effect of NACT.
