文拉法辛治疗老年高血压合并焦虑的临床研究

摘　　要：目的探讨文拉法辛在老年高血压合并焦虑患者中的疗效和安全性。方法老年高血压患者根据汉密顿焦虑量表(HAMA)评分,评分阳性分成两组(文拉法辛组和氟西汀组),评分阴性为对照组。3组均口服氨氯地平降压治疗,每次5 mg,每日1次,文拉法辛组同时加用文拉法辛,75 mg/d。氟西汀组同时加用氟西汀,20 mg/d。对照组加用安慰剂治疗。疗程均为6周。结果3组降压有效率差异无统计学意义;文拉法辛组和氟西汀组患者焦虑状态均有明显改善,疗效差异无统计学意义;文拉法辛组HAMA评分从第1周末起就明显下降,早于氟西汀组;文拉法辛组和氟西汀组不良反应比较,差异无统计学意义。结论文拉法辛在老年高血压合并焦虑患者的治疗中,其疗效与氟西汀相当,起效快于氟西汀,不良反应轻微,为安全有效的药物。Objective To observe the efficacy and safety of Venlafaxine in old patients with primary hypertension. Methods Elderly hypertensive patients were divided into three groups. The group Venlafaxine and Fluoxetin were those who got positive result during HAMA scale rating and those who got normal scores belong to comparative group. All the patients were treated with oral dose of Amlodipine 5mg/day,while the patients took Venlafaxine 75mg/ day in group Venlafaxine, those took Fluoxetin 20mg/day in the group Fluoxetin. The period of treatment was 6 weeks. Results There was no significant difference in efficacy of anti-hypertension in three groups. The groups Venlafaxine and Fluoxetin showed similar effects,with no significance. The scores of HAMA decreased significantly at the end of first week in the Venlafaxine group,was earlier than that in the Fluoxetin group. There was no significant difference in side effects between the Venlafaxine and Fluoxetin group. Conclusion Venlafaxine is an effective and safe drug for the treatment of anxiety in elderly hypertensive patients with anxiety.
