摘  要:目的通过观察急性肺损伤(ALI)大鼠血液和肺组织中IL-6、IL-10、TNF-α、NF-κΒ等炎性细胞因子的变化,探讨抑制炎症反应在治疗急性肺损伤中的作用。方法应用气管内滴入内毒素(LPS)复制大鼠急性肺损伤模型。将48只大鼠随机分成4组:对照组(N组)、LPS组、N-乙酰半胱氨酸(NAC)治疗组(NAC组)、NAC联合地塞米松(Dex)治疗组(NAC+Dex组),采用免疫印迹法(Western Blotting)检测NF-κΒ的含量,采用RT-PCR检测肺组织中IL-6、IL-10、TNF-α、NF-κΒmRNA的表达,并采用放免检测血清及支气管肺泡灌洗液(BALF)中的IL-6、TNF-α的含量。结果LPS组大鼠肺病理可见明显的肺损伤表现,NAC组及NAC+Dex组肺组织病理改变明显减轻;NAC组及NAC+Dex组肺NF-κB,肺IL-6、IL-10、TNF-α、NF-κΒmRNA表达水平及血清和BALF中的IL-6、TNF-α含量明显高于N组(P<0.01),而明显低于LPS组(P<0.05)。结论NAC、Dex等可以抑制炎症反应,减少炎性介质的释放从而减轻肺损伤的程度,达到防治急性肺损伤的目的。Objective To investigate the treatment effect of anti-inflammatory response on the expressions of IL-6 ,IL-10,TNF-α,NF-κB mRNA in the lung and the levels of IL-6,TNF-α in serum and BAI,F in rats with lipopotysaccharide-induced acute lung injury(ALI). Methods A total of 48 Spradue Dawley rats were divided into 4 groups randomly: normal saline control group(N group), lipopolysaccharid group (LPS group), N-acetylcysteine group (NAC group), NAC associate with dexamethasone group (NAC+Dex group). The levels of TNF-α,IL-6 were determined by radioimmunoassay. The expresions of IL-6,IL-10,TNF-α,NF-κB mRNA were detected by RT- PCR,NF-κB were determined Western-Blotting. Results Compared with those in the nomml saline control group,the expression levels of IL-6,IL-10,TNF-α,NF-κB mRNA and NF-κB in the injured lungs and the levels of IL-6,TNF-α in serum and BALF increased signifieantly(P〈0.01) ; Compared with those in the LPS group,NAC group and NAC+Dex group decreased significantly (P〈0.05). Conclusion N-acetylcysteine and dexamethasone can inhibit the production of these inflammatory cytokines in vivo and the mRNA expression in the lung tissues. Anti-inflammatory response may prevente acute lung injury.
