HLA-ABC、HLA-DR在肝硬化门静脉高压症脾脏中的表达及意义

摘　　要：目的探讨人类白细胞ABC抗原(HLA-ABC)、人类白细胞DR抗原(HLA-DR)在肝硬化门静脉高压症脾脏中的表达及临床意义。方法用免疫组化Envision法,检测45例肝硬化门静脉高压症(研究组)和15例外伤性脾破裂(对照组)患者脾脏中HLA-ABC及HLA-DR的表达情况。结果研究组HLA-ABC、HLA-DR阳性细胞率分别为(31.04±8.21)%和(7.57±2.57)%,显著低于对照组的阳性细胞率(46.54±6.14)%和(13.46±1.89)%(P<0.01)。HLA-ABC、HLA-DR的表达随巨脾纤维化分级和肝功能Child-pugh分级的加重而下降,且均呈负相关;而与年龄、性别、上消化道出血次数及食道胃底静脉曲张程度无关。结论肝硬化门静脉高压症脾脏中HLA-ABC、HLA-DR表达下调,可能是患者脾脏免疫功能低下的原因之一;且脾脏纤维化程度和肝功能Child-pugh分级结果可能有助于患者脾脏免疫功能的评估。Objective To investigate the significance of HLA-ABC and HLA-DR expressions in the spleen of patients with cirrhosis and portal hypertension. Methods The expressions of HLAABC and HLA-DR in the spleen of 45 patients with cirrhosis and portal hypertension（experiment group） and 15 traumatic rupture（control group） patients were tested by the Envision immunohistoehemistry method. Results The positive ratios of HLA-ABC and HLA-DR expressions were （31.04±8.21） % and （7.57 ± 2.57） % in the experiment group respectively, while （46.54± 6.14）% and（13.46±1.89）% in the control group,which showed significant differences between two groups （P〈0.01）. The expressions of HLA-ABC and HLA-DR were decreased associated with the aggravation of the cirrhotic spleen levels and Child-pugh types of liver function. Its expression had association with cirrhotic spleen levels and Child-pugh types of liver function, but had no association with age,sex, the frequency of upper gastrointestinal bleeding,and the level of esophagogastric variees. Conclusion The positive ratio.of HLA-ABC and HLA-DR expressions in the spleen with cirrhotic portal hypertension were obviously lower than it is in the control group. It may be one of the reasons that the spleen immune function of those patients was lower thannormal. The result of the cirrhotic splenic levels and Child-pugh types of liver function may helpful for the evaluation of the cirrhotic splenic immune function of those patients.
