摘 要:目的利用多层螺旋CT灌注扫描技术,分析肾脏灌注图像及灌注参数变化规律,探讨CT灌注成像在肾脏良恶性肿瘤的诊断与鉴别诊断的应用价值。方法利用美国GE公司LightSpeed CT 16 splice或64splice多层螺旋CT灌注扫描技术对正常志愿者6例及肾脏肿瘤(良恶性)患者25例的CT灌注参数值和参数图进行分析。结果①肾细胞癌(15例)肾皮质BF、BV明显高于正常肾组织(P<0.05)。肾错构瘤(10例)肾髓质BF及肾细胞癌肾髓质BF低于正常肾组织。②肾细胞癌及肾错构瘤MTT明显缩短,其中肾细胞癌MTT短于肾错构瘤(P<0.05)。③肾细胞癌BF、BV高于肾错构瘤(P<0.05)。④BF,BV,MTT具有统计学意义(P<0.05)。结论多层螺旋CT灌注扫描技术能够较好地反映正常肾组织及肾脏良恶性肿瘤的血流动力学变化,为肾脏良恶性肿瘤的诊断与鉴别诊断提供可靠的依据。Objective Using the multi-slice CT perfusion scanning technique,the analytical study kidney perfusion image and the perfusion parameter' s change rule, discusses the CT perfusion image formation in the kidney benign tumor and the malignant tmnor differential diagnosis clinical practice value. Methods Irrigation scanning technique (positive malignancy) the patient 25 example's CT irrigation parameter values and the parameter chart carries on the analysis using American GE Corporation LightSpeed CT 16 splice or 64splice the multi-layered screw CT to the normal volunteer 6 examples and the kidney tumors. Results ①Renal carcinoma kidney cerebral cortex BF, BVis higher than the renal angipleiomyolipoma organization obviously (P 〈 0.05). Renal angipleiomyohpoma kidney cerebral cortex BF and renal carcinoma kidney medullary substance BF is lower than the normal kidney organization. ②Renal carcinoma and renal angipleiomyolipoma MTT obvious reduction, its wolffian body renal carcinoma MTT short in renal angipleiomyolipoma( P 〈 0.05) . ③Renal carcinoma BF is higher than the renal angipleiomyolipoma( P 〈 0.05). ④BF, BV, MTT have statistics significance ( P 〈 0.05). Conclusion The muhi-multi-slice CT perfusion scanning technique can reflect that well the normal kidney organization and the kidney positive malignant tumor's blood stream dynamics change, provides the reliable basis for the kidney positive malignant tumor's diagnosis and the differential diagnosis.
