抑郁症患者成人依恋与防御方式研究

摘　　要：目的:通过成人依恋理论对抑郁症患者防御机制进行研究及对抑郁症的病理心理学机制进行探讨。方法:对50例抑郁症患者和60例正常对照用亲密关系经历量表和防御方式量表进行评定比较。结果:(1)抑郁症组在依恋焦虑维度上得分明显高于对照组(80·30±18·76,65·35±17·01,P<0·005);抑郁症组在依恋回避维度上得分高于对照组(60·32±16·08,52·05±14·53,P<0·05)。(2)抑郁症组与正常对照组比较多采用不成熟防御机制(141·6±36·19,105·52±29·60,P<0·005)和中间型防御机制(186·92±26·41,173·15±31·16,P<0·05)(3)对照组在依恋焦虑维度和回避维度均与不成熟的防御方式(r=0·480,P<0·005,r=0·483,P<0·005),中间型防御方式正相关(r=0·423,P<0·005,r=0·353,P<0·05)。(4)抑郁症组依恋焦虑维度与不成熟防御机制(r=0·588,P<0·005)、中间型防御机制(r=0·497,P<0·005)呈正相关。结论:抑郁症患者在依恋焦虑维度上呈现高焦虑的特点,在防御方式上多采用不成熟和中间型的防御方式,抑郁症患者的不成熟的防御方式、中间型防御方式与依恋焦虑维度呈正相关。不安全的依恋模式可能是抑郁症发病的一个主要的病理心理学机制。Objective：To study the defense style and adult attachment of depression patient and the psychopathology of depression by adult attachment theory.Methods：Fifty depression patients were accessed with Experiences in Close Relationships Scale and Defensive style Questionnaire,which were compared with sixty normal controls.Results：（1）The score of attachment anxiety dimension in the depression group was higher than that of the control group（80.30±18.76,65.35±17.01,P〈0.005）.The score in attachment avoidance in depression was higher than that of the control group（60.32±16.08,52.05±14.53,P〈0.05）.（2）The score of immature defense mechanism（141.6±36.19,105.52±29.60,P〈0.005）and intermediate defense mechanism in depression group was higher than that of the control group（186.92±26.41,173.15±31.16,P〈0.05）.（3）The attachment anxiety dimension and avoidance dimension were both positively correlated with the immature defense style（r=0.480,P〈0.005,r=0.483,P〈0.005）and the intermediate defense style（r=0.423,P〈0.005,r=0.353,P〈0.05）and in the control group.（4）The attachment anxiety dimension was positively correlated with the immature defense style（r=0.588,P〈0.005）and the intermediate defense style（r=0.497,P〈0.005）in the depression group.Conclusion：The attachment style was highly anxiety in the depression group,and the depression patients mostly used immature and intermediate defense style.The immature defense style and intermediate defense style were positively correlated with the attachment anxiety dimension.The unsafe attachment style may be the major influence factor in the process of depression.
