摘 要:目的研究传导性角膜成形术(CK)术后早期远近视力变化、暗视/明视下对比敏感度函数(CSF)变化及眩光刺激对CSF的影响,评估患者术后早期的视觉质量。方法对24例有老视症状者单眼行CK治疗以改善近视力,其中11例伴轻度远视者另一眼同时手术以提高远视力,并分别于术前及术后1、3、6个月测量2种模拟背景光(暗视/明视)下的CSF和暗视/明视下有眩光刺激下的CSF。结果双眼术后各空间频率CSF较术前差异无统计学意义(P>0.05)。双眼明视下手术前后眩光刺激对各空间频率的CSF差异均无统计学意义(P>0.05),暗视下眩光刺激明显降低了术后3个月的1.5、3、6个月1.5cpd的CSF(P<0.05)。术眼暗视下术后1、3、6个月眩光刺激引起的CSF的降低在低频区(1.5cpd、3cpd),差异有统计学意义(P<0.05)。结论CK术后,明视/暗视下各空间频率CSF均无明显变化。眩光刺激对CSF的影响主要集中在暗视下中、低频率。Objective Conductive keratoplasty(CK) is a safe and an effective surgery of correcting presbyopia which makes both eyes form monoblepsia.There are many researches about changes of visual acuity and diopter about CK,but few reports of contrast sensitivity and glare after CK were seen.This paper was to investigate the early change of distance/near vision,contrast sensitivity function(CSF) and the effect of glare on CSF after CK under the mesopic and photopic conditions and evaluate the early postoperative functional visual performance. Methods A total of 35 eyes from 24 patients with presbyopic symptoms received CK to improve near vision in lateral eyes.In addition,11 fellow hyperopic eyes were treated to improve distance vision.CSF under the photopic and mesopic conditions was tested and the influence of glare on CSF was assessed at preoperation,postoperative 1 month,3 months and 6 months. Results No significant difference was seen in distance/near vision before and after CK(χ2=5.39,P=0.146),but the near vision was significantly elevated after CK(χ2=41.53,P=0.000).There was no significant difference in binocular contrast sensitivity in all spatial frequencies between preoperation and postoperation(P〉0.05).Under the photopic condition,the effect of glare on CSF was not conspicuous before and after surgery(P〉0.05).Glare resulted in a significant decrease in mesopic CSF for 1.5 and 3 cpd spatial frequencies at 3 months postoperatively(P〈0.05).The difference in low spatial frequencies(1.5 and 3cpd) at 1,3 and 6 months postoperatively was statistically significant in the effect of glare on mesopic CSF in all operative eyes. Conclusion There is not obvious change in CSF in all spatial frequencies after CK whether photopic or mesopic condition.Glare decrease the mesopic CSF mainly in intermediate and low spatial frequency postoperatively.
