摘 要:目的探讨采用附着体修复单侧上颌骨缺损的可行性。方法在已完成的环氧树脂单侧上颌骨缺损牙颌模型上,分别制作传统卡环修复体和附着体修复体。采用光弹应力分析法,对两种修复体在载荷条件下的应力分布情况进行分析和比较。结果传统卡环修复体与附着体修复体在腭部应力分布无明显差异,健侧后牙受力均匀;与传统卡环修复体比较,附着体修复的中切牙近中牙槽嵴应力条纹级数相对较高。结论附着体修复体健侧后牙的应力分布均匀,符合生物力学要求;其对于中切牙和中切牙近中牙槽嵴应力较集中,可通过增加基牙数目的联冠设计提高支持作用和对抗转动扭转力。Objective To explore the feasibility of prosthesis with attachment for unilateral maxillary defect. Methods The unilateral maxillary defect models with traditional clasp and attachment were prepared respectively. Photoelastic analysis was conducted to compare the stress distribution with load between the two restorations. Results There was no significant difference in stress distribution in palate between restoration with traditional clasp and that with attachment, and the stress was well distributed on the posterior teeth of the healthy side. Compared with restoration with traditional clasp, the stress fringe value of mesial alveolar bone of central incisor was relatively higher in restoration with attachment. Conclusion For restoration with attachment, the stress is well distributed on the posterior teeth of the healthy side, with a favourable biomechanical characteristics. The stress mainly concentrates on central incisor and mesial alveolar bone of central incisor, and modified design with more abutments may improve the role of support and combact the torsional force of rotation.
