摘 要:目的探讨富含半胱氨酸的酸性分泌蛋白(SPARC)在乳腺浸润性导管癌组织中的表达及意义。方法以乳腺浸润性导管癌手术切除标本作为研究对象,运用免疫组化EnVision二步法检测癌组织(乳腺浸润性导管癌组,n=61)中SPARC的表达,以远离肿瘤的乳腺组织作为对照(对照组,n=32)。对与SPARC表达可能相关的因素进行分析。结果对照组上皮细胞中SPARC阳性表达率为31.3%,间质细胞未见SPARC表达(0%)。SPARC在乳腺浸润性导管癌组的癌细胞和间质细胞中均有表达,在间质细胞的阳性表达率为70.5%,与相应对照组比较差异有统计学意义(P<0.001)。伴淋巴结转移乳腺浸润性导管癌肿瘤细胞中SPARC阳性表达率显著高于不伴淋巴结转移者(46.7%vs 22.6%),两者比较差异具有统计学意义(P<0.05);乳腺浸润性导管癌中SPARC表达与患者年龄和肿瘤大小、组织学分级、TNM分期等均无明显相关性(P>0.05)。伴有癌转移淋巴结的SPARC阳性表达与原发肿瘤的临床分期有关(P<0.05)。结论SPARC在乳腺浸润性导管癌肿瘤间质细胞中的表达显著增高,提示SPARC可能通过间质细胞对肿瘤细胞的调控参与乳腺癌的发生;同时肿瘤细胞SPARC表达与乳腺癌转移相关。确切机制尚待进一步深入研究。Objective To investigate the expression and significance of secreted protein acidic and rich in cysteine (SPARC) in invasive ductal carcinomas of breast. Methods The resected specimens from invasive duetal carcinomas of breast were selected. The expression of SPARC in carcinoma tissues ( invasive ductal carcinomas of breast group, n = 61 ) was detected by imnmnohistochemistry EnVision method, and the breast tissues away from carcinoma were served as controls ( control group, n = 32). The possible factors that were related to the expression of SPARC were analysed. Results The positive expression rate of SPARC in epithelial cells of control group was 31.3% , while there was no expression in stromal cells. There was expression of SPARC in both the carcinoma cells and stromal cells in invasive ductal carcinomas of breast group, with a positive expression rate of 70.5% in stromal cells, significantly different from that of the corresponding control group (P 〈0. 001 ). The positive expression rate of SPARC in carcinoma cells in invasive ductal carcinomas of breast with lymph node metastasis was significantly higher than that without lymph node metastasis (46.7% vs 22.6% , P 〈 0.05). There was no significant association between the expression of SPARC in invasive ductal carcinomas of breast and the factors such as age, tumor size, histological grade and TNM stage(P 〉0.05). The positive expression of SPARC with lymph node metastasis was related to the clinical stage of primary tumor ( P 〈 0.05 ). Conclusion The expression of SPARC in stromal cells in invasive ductal carcinomas of breast is significantly increased, indicating that SPARC may participate in the pathogenesis of breast carcinoma by regulation of carcinoma cells through stromal cells. However, the precise mechanism needs further research.
