不同金属烤瓷冠对龈沟液的影响

摘  要:目的通过检测实验犬牙的菌斑指数(plaque index,PLI)、探诊深度(probing depth,PD)、龈沟内出血指数(sulcus bleeding index,SBI)、龈沟液的量(gingival crevicularfluid,GCF)以及龈沟液碱性磷酸酶(alkaline phosphatase,ALP)水平,比较镍铬合金烤瓷冠和镀金镍铬合金烤瓷修复对龈沟液的影响。方法对4只当地犬的40颗犬牙行烤瓷冠修复。一组为普通镍铬合金烤瓷冠修复,另一组为镀金镍铬合金烤瓷修复,每组20颗烤瓷冠。分别于备牙前、戴牙后1个月、3个月,检测实验牙PLI、SBI、PD、GCF以及ALP的水平。结果1个月后,镍铬合金烤瓷修复组、镀金镍铬合金烤瓷修复牙组的PLI、PD、SBI、GCF、GCF-ALP水平与备牙前均无显著差异(P>0.05),3个月后镍铬合金烤瓷修复组的GCF、GCF-ALP的水平升高,与备牙前及镀金镍铬合金组均有显著差异(P<0.05);镀金镍铬合金烤瓷修复组与备牙前各项指标无显著差异(P>0.05)。结论镍铬合金烤瓷修复体金属表面镀金处理,方法简便、成本较低、对牙周组织刺激小。Objective To investigate the effect of Ni-Cr metal ceramic crown and gold-plating Ni- Cr metal ceramic crown in gingival crevicular fluid by measuring PLI,PD, SBI,the weight of GCF and the amount level of ALP in gingival crevicular fluid (GCF) after PFM restoration. Methods 4 local dogs were utilized in this study. After 2 weeks cornmenceing oral hygiene procedures through supragingival scaling and subgingival scaling, 40 teeth for dogs were prepared for receiving Ni-Cr metal ceramic crown and gold-plating Ni-Cr metal ceramic crown. The restoration of group 1 had no treatment while those in group 2 were treated with gold-plating technique in the base metal alloy. Clinical parameters were recorded on all test teeth immediately before tooth preparation and after lmonth,3 months of cementation of PFM crowns. Clinical parameters included PLI,PD,SBI, the weight of GCF and the level of ALP in GCF. Results Compared to pre-restoration, all indexes had no conspicuous difference among two groups after 1 month. Ni-Cr metal ceramic crown group had no difference with gold-plating technique group too. The weight of C,-CF and the level of ALP of the Ni-Cr metal ceramic crown group had conspicuous difference with gold-plating technique group and pre-restoration respectively. However, gold-plating technique group had no difference with pre-restoration after 3 months. Condmion he goldplating technique is plating the gold on the surface of Ni-Cr porcelain restoration to prevent corrode. This technique has the advantages of simple in method, cheaper, no harmful to gingival, and good effect.
