摘 要:目的观察经导管肝动脉化疗栓塞术联合超液化碘油+细胞因子对兔移植性肝肿瘤的治疗作用。方法采用新西兰大白兔VX2肿瘤株移植到肝脏的方法制作肝肿瘤模型30只,根据治疗方案的不同随机平均分为生理盐水对照组(G1),常规TACE组(G2),TACE+肝动脉灌注细胞因子组(G3),TACE+脾动脉灌注细胞因子组(G4)及TACE联合超液化碘油+细胞因子组(G5)共5组,每组6只,进行肝动脉插管治疗。观察各组肿瘤生长、坏死情况以及肿瘤组织内增殖细胞核抗原的表达。结果①治疗后第7天,G5肿瘤增长率显著低于G1、G2、G3、G4(P<0.05);G3、G4与G2比较(P<0.05);G5生长抑制率则显著高于G2、G3、G4(P<0.05),G3、G4与G2比较(P<0.05)。②G5肿瘤坏死范围与G1、G2、G3、G4相比较有显著性差异;G3、G4与G2相比较无显著性差异(P(0.05)。③G3组、G4组、G5组的肿瘤组织内增殖细胞核抗原标记指数显著小于对照组G1和G2组(P<0.01);而G2组与G1组相比较,无显著意义(P>0.05);G5组与G3组、G4组比较(P<0.05)。结论TACE联合超液化碘油+细胞因子免疫疗法是一种治疗肝癌的有效方法。Objective To observe the therapeutic effects of TACE combined with immune therapy on transplanted liver neoplasm in rabbits. Methods Thirty New Zealand White rabbits with transplanted VX2 liver cancer were randomly divided into 6 groups according to the modes of treatment ( 6 rabbits each group) :including normal saline as control( G1 ) ,TACE alone( G2) ,TACE + injection of cytokine via hepatic artery (G37 ,TACE + injection of cytokine via splenic artery (G4) and TACE combined with lipiodol emulsion + cytokine (G5). The therapeutic effects were observed including the changes of tumors' volume, the necrosis of tumors and the expression of proliferating cell nuclear antigen( PCNA) inside the tumors. Results ①7th day after treatment ,the growth rate (GR) of tumors in G5 was significantly lower than that in G1, G2, G3 and G4 ( P 〈 0.05 ) , and the G3, G4 was lower than that in G2. The inhibition of tumor growth in G5 was significantly higher than that in G2, G3 and G4 ( P 〈 0.05 ). ②The necrosis of tumors in G5 was significantly more extensive than those in G1, G2, G3 and G4( P 〈0. 05). However, there was no significant difference between the extent of necrosis in G3, G4 and G2 (P 〉0. 057. ③PCNA labeling indexes(PCNA LI) in G3, G4 and G5 were significantly lower than that in G1 and G2 (P 〈 0.01). There was no significant difference between G1 and G2. PCNA LI in G5 was significantly lower than that in G3 and G4 ( P 〈 0.05 ) . Conclusion TACE combined with lipiodol emulsion + cytokine is a effective therapeutic method for hepatic carcinoma.
