摘 要:目的探讨宫颈癌患者围手术期血清免疫抑制酸性蛋白(IAP)和T淋巴细胞rDNA转录活性的变化及其临床意义。方法分别用单向免疫扩散法和核仁形成区相关蛋白银染技术测定64例宫颈癌手术前后血清IAP和外周血T淋巴细胞rDNA转录活性的改变,并与健康对照组比较分析。结果宫颈癌组治疗前较对照组血清IAP增高[(688.5±53.3)μg/mLvs(479.1±48.4)μg/mL],外周血T淋巴细胞rDNA转录活性降低[(6.24±0.64)%vs(8.33±0.77)%],与对照组比较差异有统计学意义(P<0.01)。行根治性手术组(35例)术后血清IAP显著性降低[(511.3±54.8)μg/mLvs(668.2±44.2)μg/mL](P<0.01),外周血T淋巴细胞rDNA转录活性显著性增高[(6.96±0.34)%vs(6.12±0.51)%](P<0.01);而姑息性手术组(29例)血清IAP和外周血T淋巴细胞rDNA转录活性则无显著性变化[(657.2±58.3)μg/mLvs(674.9±48.7)μg/mL;(6.12±0.41)%vs(6.18±0.37%)](P>0.05)。结论提示宫颈癌患者血清IAP和T淋巴细胞rDNA转录活性的变化与肿瘤的浸润转移和病程有关,检测宫颈癌患者血清二者的变化,有助于估计患者的治疗效果和预后。Objective To study the changes of serum immunosuppressive acidic protein (IAP) and T-lymphocytes rDNA transcription activity in peripheral blood of patients with cervical cancer before and after operation. Methods The level of IAP and T-lymphocytes rDNA tran- scription activity in peripheral blood of 64 cases with cervical cancer before and after operation were measured by one-directional immunodiffusion test and a cell image analysis of Ag-NORs respectively and compared them with that of health controls. Results In the patients with cervical cancer before operation the level of IAP was higher[ (688.5 ± 53.3 ) μg/mL vs (479.1 ± 48.4) μg/mL] while T-lymphocytes rDNA transcription activity of PBL was lower[ (6.24 ± 0.64) % vs ( 8.33 ± 0.77 ) % ] than that of health controls ( P 〈 0.01 ). The levels of IAP was significantly decreased [ (511.3 ± 54.8)μg/mL vs (668.2 ± 44.2) μg/mL] in 35 cases with cervical cancer after radical resection(P 〈 0. O1 ), while the level of T-lymphoeytes rDNA transcription activity was significantly increased [ (6.96 ± 0.34) % vs (6.12 ± O. 51 ) % ] ( P 〈 0.01 ). But there was not significant changes of the levels of lAP [ (657.2 ± 58.3 ) μg/mL vs ( 674.9 ± 48.7 )μg/mL ] and T-lymphocytes rDNA transcription activity [ (6.12 ± 0.41 ) % vs (6.18 ±0.37% ) ] in 29 cases with cervical cancer after palliative operation( P 〉 0.05). Conclusion The levels of IAP and T-lymphocytes rDNA transcription activity are closely related to tumor invasion, metastasis and clinical stage (TNM). Dynamic determination of the levels of IAP and T-lymphocytes rDNA transcription activity may be an important index for evaluation of invasion, metastasis, efficacy and prognosis for the patients with cervical cancer.
