血管内皮生长因子与卵巢恶性肿瘤的诊治及其病情监测的关系

摘　　要：目的探讨血管内皮生长因子(VEGF)与卵巢恶性肿瘤的诊治和预后之间的关系。方法以本院近4年收治的恶性卵巢肿瘤患者31例为研究对象,并与同期收治的34例良性卵巢肿瘤患者对照。采用酶联免疫法动态检测血清VEGF水平,内胚窦瘤病例同时放免测定血清CA125水平。结果①卵巢恶性肿瘤术前血清VEGF水平明显高于卵巢良性肿瘤和健康妇女,晚期病例明显高于早期病例,均有极显著差异;②血清VEGF与CA125水平均随卵巢恶性肿瘤病情控制情况而变化,且两者呈高度正相关。动态监测血清VEGF水平诊断卵巢恶性肿瘤及其分期和预后的准确性均在80%以上。结论血清VEGF水平与卵巢恶性肿瘤的生长、转移和预后密切相关,具有肿瘤标志物特性,可作为卵巢恶性肿瘤的诊治和病情监测指标之一。Objective To study the roles of vascular endothelial growth factor （VEGF） on the diagnosis and monitoring of ovarian malignant tumour. Methods There were 31 patients with ovarian malignant tumor act as study group, and 34 patients with benigh ones as control group near 3 years. Serum VEGF levels were continuously determined by enzymoimmunoassay before and after operation, serum CA 125 in patients with endodermal sinus malignant tumour were monitored by radioimmunoassay. Results（1）Serum VEGF levels before operation in patients with ovarian malignant tumour were significantly higher than those with benigh ones and the healthy contMs. Serum VEGF levels in patients with advanced stage ovarian malignant tumour wrer apparently higher than those at early stage; （2）Serum VEGF levels had a positive correlation with the CA125 levels, and they progressively dropped under the control of illness. Serum VEGF levels could be used to diagnose ovarian malignant tumour, determine their stages and outcomes with an accuracy of 80% by dynamic monitoring. Conclusion Serum VEGF with the characteristics of tumor markers shows a strong relationship with growths,metastases,outcomes of ovarian malignant tumour. So it can be one of the parameters of diagnosis and illness monitoring in ovarian malignant tumour.
