颧骨颧弓缩小及下颌角截骨术27例临床体会

摘　　要：目的:通过手术改善部分方脸畸形患者面部外观,观察手术效果,探讨有效的手术方式。方法:对27例要求行面部轮廓整形的患者施行口内联合耳前切口颧骨颧弓缩小术及下颌角截骨术,纠正患者的下颌角肥大及颧骨颧弓的肥大。结果:所有患者术后随访3月,均恢复良好,术后面部轮廓改善满意。其中5例术后出现下唇麻木,术后3月左右自行恢复。结论:颧骨颧弓缩小及下颌角截骨术对方脸患者面部轮廓改善效果良好,同时面部神经血管丰富,术者应熟悉解剖,不断积累手术经验及操作技巧才能达到满意的手术效果。Objective To observe the operation results and to explore the effective method by improving the shape of square face through operation. Methods The 27 patients who asked for improving facial contour were treated by reduction plasty of mala and zygoma through intra-oral accompanied minor pre-auricular incision and the mandibular angel ostectomy in order to improve the shape of square face. Results The face skeleton of all patients was improved with satisfaction. All the patients obtained good result during the follow-up period of three months. Of them there were 5 cases with numbness of labial angle and it had been cured at 3 months after operation. Conclusion Reduction plasty of mala and zygoma and mandibular angel ostectomy can improve the facial shape properly. The surgeon must know the anatomy well and accumulate experience and surgical skills continually in order to obtain the satisfactory operation results because of rich blood supply and nerve in face.
