玻璃体内注射先锋霉素Ⅵ对视网膜酶组织化学影响

摘　　要：①目的探讨先锋霉素Ⅵ玻璃体腔内注射的安全剂量。②方法以不同浓度的先锋霉素Ⅵ（１００，２００，２５０，３００，４００μｇ）行兔眼玻璃体腔内注射，用酶组织化学染色法测定注射后不同时间（１，３，７ｄ）视网膜琥珀酸脱氢酶（ＳＤＨ）和乳酸脱氢酶（ＬＤＨ）活性的变化，并观察对视网膜组织结构及超微结构的影响。③结果随用药剂量增大，视网膜内ＳＤＨ与ＬＤＨ活性均逐渐降低；各剂量组视网膜内ＳＤＨ与ＬＤＨ活性分别于术后３ｄ和１ｄ降至最低，然后开始恢复，至手术７ｄ后１００和２００μｇ组恢复至正常，其余剂量组仍低于正常。组织学检查显示１００和２００μｇ剂量组视网膜出现水肿，其余剂量组均出现细胞变性坏死、杆锥体脱失等改变。④结论先锋霉素Ⅵ玻璃体腔内注射的安全剂量为２００μｇ．Objective To study the safe dose of intravitreal injection of cephalosporins. Methods After injecting different doses of cephalosporins(100, 200, 300, 400μg) into vitreous body of rabbits, we studied the active change of retinal enzymes(SDH, LDH) at different time and observed the histological and ultrastuctural changes of retinae. Results Along with the increase of cepholosporin concentration, the activities of SDH and LDH gradually decreased and reached the lowest level on the 1st and 3rd day after operation and then their activities began to recover. On the 7th day after operation, the activities of the 100μg group and 200μg group returned to normal, but the other groups still lower than normal. Histochemical observation showed that retinae in the 100, 200 μg groups appeared edema, in the 250μg, 300μg and 400μg groups, the retinal cells were degenerative, necrotic and cones and rods disappeared. Conclusion The safe dose of intravitreal injection of cephalosporins is 200μg.
