辅助鼻内镜微创手术治疗鼻咽纤维血管瘤的价值

摘　　要：目的探讨选择性栓塞联合鼻内镜治疗鼻咽纤维血管瘤(JNA)的临床价值。方法收集22例确诊为鼻咽纤维血管瘤患者,其中13例行术前选择性栓塞联合鼻内镜微创治疗,9例行直接手术切除,统计患者治疗过程的平均出血量、输血量及手术时间。结果联合治疗组平均术中出血量为(400±200)mL,而输血量为(350±250)mL;而直接手术切除组术中出血量及输血量分别为(1210±400)L和(1180±500)mL。联合治疗组及直接手术切除组的手术时间分别为(95±10)min和(145±20)min。结论选择性栓塞联合鼻内镜微创手术对鼻咽纤维血管瘤疗效确切,此技术能有效减少术中出血量,操作简单,费用低廉,是一种有效的鼻咽纤维血管瘤治疗方法,值得在临床推广。Objective To approach the clinical value of ultraselection embolism combined with therapeutic endoscopy. Methods 22 cases patients diagnosed as JNA were divided into 13 cases of therapeutic alliance group and 9 cases of exairesis group,and collect the mean volume of blood loss, transfusion volume and operation time. Results In therapeutic alliance group, the mean volume of blood loss and transfusion volume is （400 ± 200）mL and （350 ± 250）mL, but the volume is （ 1210 ± 400）mL and （ 1180±500）mL in exairesis group. On the other hand, the operation time in therapeutic alliance group and exairesis group are （95 ± 10）min and （145 ± 20）min. Conclusion There are certain curative effect by ultraselection embolism combined with therapeutic endoscopy to JNA,and it can decrease the blood loss and operation time, and it＇s easy working and cheaper. The therapeutic alliance method is a effective treatment, and is valuable to promote in clinic.
