摘  要:背景:传统观念认为内皮修复再生是临近部位成熟内皮细胞迁移、增生的结果。近期研究提示,体内循环血池中的内皮祖细胞参与了损伤部位的内皮修复,而骨髓干细胞中含右大量的内皮祖细胞。目的:观察兔骨髓干细胞植入后,行自体静脉移植的静脉桥血管再内皮化及内膜增生的情况。设计、时间及地点:开放性实验,于2007-05/2008-05在北华大学附属医院内科实验室完成。材料:清洁级新西兰大白兔23只,随机分为细胞移植组13只,空白对照组10只。方法:两组兔均建立自体静脉移植模型,麻醉后颈前正中切口,游离出左侧颈外静脉及颈总动脉,在颈总动脉的上下两端阻断血流后,横断血管,取左侧的颈外静脉4.0~5.0cm,上下倒向后置于同侧颈总动脉之间,采用间断外翻法行端端吻合。细胞移植组于造模后3d抽取自体骨髓,淋巴细胞分离液法提取单个核细胞,行荧光染料DAPI标记后,经耳缘静脉注入100μL细胞悬液(6×108个细胞),培养4周。空白对照组经耳缘静脉注入100μL磷酸盐缓冲液。主要观察指标:静脉移植段血管内皮的完整性、内膜厚度及内膜一氧化氮合酶活性。结果:与空白对照组比较,细胞移植组的移植段血管内皮细胞覆盖率明显升高(t=2.654,P<0.05);内膜厚度明显降低(t=3.474,P<0.05);内膜一氧化氮合酶活性明显升高(t=2.604,P<0.05)。结论:兔骨髓干细胞植入后,可以促进自体移植静脉桥血管的再内皮化,并抑制内膜的过度增生。BACKGROUND: Traditional concept thought that reparative regeneration of endothelium is the result of migration and proliferation of mature endothelial cells in close parts. Present studies have confirmed that endothelial progenitor cells in circulation blood pool participates in endothelial reparation in the injured regions, as well as a large number of endothelial progenitor cells are found in bone marrow stem cells (BMSCs). OBJECTIVE: To observe re-endothelialization and intimal proliferation in vein grafts following rabbit BMSC transplantation. DESIGN, TIME AND SETTING: The opening experiment was performed at the Laboratory of Department of Internal Medicine, Hospital Affiliated to Beihua University from May 2007 to May 2008. MATERIALS: Totally 23 clean New Zealand rabbits were randomly divided into cell transplantation group (n=13) and blank control group (n=10). METHODS: Rabbit models of autogenous vein graft were generated in both groups. After anaesthesia, a median incision was made at the neck to dissociate external jugular vein and common carotid artery of the left side. Following blood flow was blocked at the upper and down sides of the common carotid artery, blood vessels were transected to collect 4.0-5.0 cm external jugular vein at the left side, and placed among the common carotid artery at the same side after inversion. End-to-end anastomosis was performed using discontinuous ecstrophy. Bone marrow was extracted at 3 days after model induction in the cell transplantation group, and mononuclear cells were harvested using lymphocyte isolation. Following treatment of fluorescent dye DAPI labeling, BMSCs (6×10^8cells)/100 μL were transplanted into the ear vein, and incubated for 4 weeks. 100 μL phosphate buffer saline was infused into rabbits in the blank control group.
