血清CEA,CA125,NSE,CYFRA21-1联合检测对肺癌的诊断价值

摘　　要：目的探讨血清癌胚抗原(CEA)、糖类抗原125(CA125)、细胞角蛋白片段21-1(CYFRA21-1)、神经元特异性烯醇化酶(NSE)联合检测对肺癌的诊断价值。方法采用电化学发光法对78例肺癌(其中鳞癌30例,腺癌28例,小细胞癌20例)、45例肺部良性病患者和36例健康人血清进行分析。结果肺癌患者血清4种肿瘤标志物含量明显高于肺部良性疾病组(P<0.05)和健康对照组(P<0.01)。肺癌组经病理分型后,NSE在小细胞肺癌中的水平明显高于其他类型的肺癌(P<0.05);CYFRA21-1的水平在鳞癌中升高尤为明显(P<0.05);CEA的水平在腺癌中显著升高(P<0.05)。4种肿瘤标志物单项检测时,特异度较高(82.22～95.56),但敏感度不是很高(38.46～56.41)。联合检测后,可提高肺癌的阳性检出率(64.10)。结论CEA,CA125,NSE和CYFRA21-1对肺癌有一定的辅助诊断价值,且CEA,NSE和CYFRA21-1对不同组织类型肺癌均有一定诊断价值,联合检测可提高对肺癌的阳性诊断。Objective To investigate the diagnostic value of combined determination of serum carcinoembryonic antigen （CEA）,cancer antigen 125 （CA125）, cytokeratin-19-fragments （CYFRA21-1）and neuron-specific enolase （NSE） levels in serum in the diagnosis of lung cancer. Methods The levels of CEA,CA125,NSE and CYFRA21-1 in serum were measured in 78 patients （30 squamous carcinoma, 28 adeno carcinoma and 20 small ceil carcinoma）with lung cancer ,45 patients with benign lung disease and 36 health controls by using electrochemi luminescence. Results The level of all 4 tumor markers measured in serum were much higher in lung cancer group than that in benign lung disease group（P〈0.05） and controls （P〈0.01）. These tumor markers increased in different degrees among the patients in various pathological classifications. The 4 tumor markers showed higher specificity（82.22%-95.56%） and lower sensitivity（38.46%-56.41%） in single tumor marker detection,and their combined detection could significantly improve the positivity for lung cancer detection（64.10%）. Conclusion CEA,CA125,NSE and CYFRA21-1 employed separately are helpful in the diagnosis of lung cancer. Combined detection of these tumor markers would improve the positivity for diagnosis of lung cancer.
