胰腺囊实性假乳头状肿瘤2例临床病理分析

摘　　要：目的:探讨胰腺囊实性假乳头状肿瘤的临床病理特点及其与预后的关系。方法:采用组织学(HE、PAS法)和免疫组化(S-P)法,对2例胰腺囊实性假乳头状肿瘤进行标记、染色,分析其临床病理学特点与预后的关系。结果:2例患者手术后均无复发,肿瘤较大,有包膜,由实性与囊性坏死区混合组成。瘤细胞大小形态较一致,核圆形或卵圆形,核异型不明显,核分裂象罕见,可见肿瘤细胞围绕血管排列成假乳头突起,瘤细胞退变、出血,泡沫细胞和胆固醇结晶沉积。免疫组化:细胞角蛋白(CK)、波形蛋白(Vi mentin)、α-AT、雌激素受体(ER)、孕激素受体(PR)均为阳性;1例上皮膜抗原(EMA)和S-100蛋白为阳性;突触素(Syn)、神经原特异性烯醇化酶(NSE)、胰岛素、嗜铬素(CgA)、肌动蛋白(Actin)均见小灶性阳性区。结论:胰腺囊实性肿瘤具有良性临床病程,手术切除少复发,预后好。Objective： To explore the immunohistochemical features of solid-cystic pseudopapillary tumor of the pancreas （SCPT） and the relationship between clinicopathologic features and prognosis. Methods： Two cases of SCPT were studied by histologic （HE and PAS stains） and immunohistochemical methods. Resuits： Two patients were without recurrence after tumor resectionl. The tumors were larg in size, allencapsulated, and solid alternately with cysticl histologically. A uniform population of cells were seen. The nuclei were oval, round without alypia, and rate of mitotic activity were evidently. Psudopapillary structure with a fibrovascular core was remarkable. Hemorrhage, foam cells and cholesterol crystals were often found immunohistochemically, CK （＋）, Vimentin （ ＋ ）, ER（＋）, PR（＋）, EMA（ ＋ ）. Conclusion： SCPT is a clinicopathologic entity with a benign clinica coursel, without recurrence after tumor resection.
