三氧化二砷注射液治疗复发难治性非霍奇金淋巴瘤疗效观察

摘　　要：[目的]探讨三氧化二砷（As2O3）注射液治疗复发难治性非霍奇金淋巴瘤（NHL）的疗效及不良反应。[方法]25例复发难治性非霍奇金淋巴瘤（其中B细胞NHL 17例,T细胞NHL 8例）,既往均曾接受过3种或以上方案联合化疗,应用As2O3注射液0.25 mg/（kg.d）稀释后缓慢静滴4 h,每天1次,连用2周,3周为1周期,治疗2周期按WHO标准评价疗效。[结果]25例中,24例可评价疗效。CR 3例（12.5%）、PR 10例（41.67%）、SD 7例（29.17%）、PD 4例（16.67%）,有效率54.17%;其中B细胞NHL有效率64.71%（11/17）,T细胞NHL有效率28.57%（2/7）。主要毒性是骨髓抑制,多为Ⅰ-Ⅱ度,其他不良反应轻微,包括食欲下降、腹胀、恶心等。[结论]As2O3注射液治疗复发难治性非霍奇金淋巴瘤有较好的疗效,安全性好,值得临床进一步研究。[Objective] To research the efficacy and toxicity of arsenic trioxide for the treatment of patients with relapsed and refractory non-hodgkin lymphoma（NHL）.[Methods]Twenty five patients with NHL（17 B-cell NHL and 8 T-cell NHL） received three or more cycles of combined chemotherapy.Arsenic trioxide 0.25mg/kg/day was infused 4 hours every day for 2 weeks and re-infused after 1 week that one cycle consisted of 3 weeks.The efficacy after two cycles was evaluated by WHO criteria.[Results] In all patients,the effect of 24 cases was evaluated.Cross efficiency rate was 54.17% with 3 CR（12.5%）,10 PR（41.67%）,7 SD（29.17%） and 4 PD（16.67%）.The effective rate of B-cell NHL and T-cell NHL was 64.71%（11/7） and 28.57%（2/7）,respectively.The main toxicity was myelosuppression,which most of them were grade Ⅰ～Ⅱ.Other toxicities were mild including anorexia abdomen distention and nausea,etc.[Results] Arsenic trioxide for patients with NHL has better effective and less toxic effects,so it is worthy of further clinical study.
