亲属与非亲属陪护对脑卒中恢复期患者的不同影响

摘　　要：目的研究亲属陪护与非亲属陪护对脑卒中恢复期患者的不同影响。方法320例脑卒中恢复期患者,按陪护类别不同分为亲属组和非亲属组,患者均按中西医结合卒中单元模式实施综合性中西医结合治疗与护理,并对两类陪护进行健康教育。评估项目有Barthel指数(BI)、脑卒中残损评定法(SIAS)、汉密尔顿抑郁量表(HRSD)、牛津残障评分(OHS)、并发症发生率等。结果亲属组患者并发症发生率为7.0%,非亲属组为19.0%,两组比较差异有统计学意义(P<0.01),亲属组SIAS、BI出入院差值分别为(20.17±10.38)分和(22.78±11.23)分,非亲属组SIAS、BI分别为(14.75±6.31)分和(17.23±6.85)分,两组比较差异有统计学意义(P<0.05)。随访2个月OHS轻中度残障者高于非亲属组高(P<0.05)。结论亲属陪护对脑卒中患者更有益。Objective To study the influence of nursing under family members and the unfamily members on patients （pts） with stroke in convalescence stage. Methods Three hundreds and twenty pts with stroke at reeonvalescents were divided into the family member care group and the unfamily member care group. All pts were treated and nursed in TCM with western medicine stroke unit. The Barthel index （BI）, stroke impairment assessment set （SIAS）, Hamilton depression rating scale （HRSD）, Oxford handicap scale （OHS） and complications were evaluated. Results complications in family member group（7.0%） were lower than that in the unfamily member group （19.0%, P〈0.05）. The SIAS（20. 17 ±10.38 points） and BI（22.78 ±11.23 points） in the family member care group and before and after treatment were higher than that in the unfamily member care group （14. 75±6.31 points） and （17.23±6.85 points,P〈0.05）. The OHS after 2 months of discharge and the mild or moderate handicap rate in the family member care group were higher than that in the unfamily member care group （P〈0.05）. Conclusion Nursing by the family member could be more beneficial to stroke pts.
