摘　　要：目的:探讨p73和COX-2在胃腺癌中的表达及其预后意义。方法:用免疫组织化学方法检测78例胃腺癌组织中p73和COX-2的表达情况,回顾性分析p73和COX-2的表达与各项临床病理指标及预后之间的关系。结果:胃腺癌组织中p73和COX-2呈高表达。p73阳性表达与癌细胞分化程度及淋巴结转移密切相关(P<0.05);COX-2蛋白阳性表达与癌浸润深度及TNM分期密切相关(P<0.05)。5年生存率的单因素分析显示:COX-2阳性表达与胃腺癌的预后相关(P<0.01);多因素分析显示:p73和COX-2联合表达是影响胃腺癌预后的独立危险因素之一(P<0.05)。结论:p73和COX-2在胃腺癌中的表达与生物学特征密切关系,COX-2的表达可作为胃腺癌预后的一个指标,p73和COX-2联合检测对胃腺癌预后的判定有更大的临床价值。ObJective： To investigate the expression of p73 and COX-2 and their prognostic significance in gastric adenocarcinoma. Methods： The expressions of p73 and COX-2 protein were detected by immuno-histochemical assay in 78 surgical samples of gastric adenocarcinoma patients. The correlations between the expressions of p73 and COX-2 and the different clinicopathological data were analyzed retrospectively. Results： ①The p73 and COX-2 proteins in gastric adenocarcinoma tissues were over-expressed. ②The positive expression of p73 protein was highly correlated with tumor differentiation level and metastasis of local lymph node （P〈0.05）. ③The positive expression of COX-2 protein was closely correlated with depth of invasion or TNM stage （P〈0.05）.④The univariate analysis demonstrated that the 5-year survival rate in gastric adenocarcinoma patients with positive COX-2 protein expression was lower than that in patients with negative COX-2 protein expression （F〈0.01）.⑤Multivariate analysis （COX proportional hazards modeling） revealed that beth p73 and COX-2 positive expression could be one independent prognostic factors of gastric adenocarcinoma （P〈0.05）. Conclusions; The expressions of p73 and COX-2 in gastric adenoearcinoma are closely correlated with its biologic behavior. The COX-2 protein expression may serve as a fairly good prognostic factor. Furthermore, the evaluation of both p73 and COX-2 expression shows a beneficial clinical significance in judgment of the patient prognosis.
