摘　　要：目的探讨移动手持智能终端临床护理操作考核系统(clinical nursing operation exam system,CNOES)的临床应用效果。方法便利抽取某市2所三级甲等和1所二级甲等综合性医院153名内科、外科、专科及辅诊科室护士为测试对象,选取静脉输液、无菌技术、口腔护理、鼻饲静脉采血、氧气吸入、气管内或口鼻吸痰等7项操作为考核项目,分别采用传统考核与CNOES考核,比较两种考核方法完成7项操作所需成本、成绩汇总所需时间、考官及考生使用CNOES主观感受等,以评价其临床效用。结果运用CNOES考核,前期软、硬件等投入较大,所需成本高出传统纸质考试;但从后期使用成本来看,其避免了纸质消耗,具有更加长远的意义。CNOES考核成绩汇总所需时间较传统纸质考核缩短(t=13.303,P<0.01)。考官认为CNOES较传统纸质考试方法创新、给护士长工作带来便利、简化考核前准备工序、减少人力的投入,且考核较纸质考试精确,资料易查询、不易丢失,考核成绩客观;考生对使用CNOES持赞成态度。结论临床护理操作考核系统的应用实现了无纸化考核,提高了工作效率,促进了科学管理,为现代化的临床教学工作提供了必要的支持。Objective To explore the effects of applying clinical nursing operation exam system （CNOES） with mobile handheld intelligent terminal. Methods One hundred and fifty-three nurses from medical, surgical and specialty departments of 2 Grade-A and 1 Grade-B general hospitals were recruited in this study with the method of convenience sampling. All the nurses were tested 7 skills by the tranditional method and CNOES,including intravenous infusion, aspetic techniques, oral care,nasogastric feeding and so on. The cost, time to collect scores, examiners and examinees＇ subjective perception about using＇ CNOES were compared between two exam methods so that the clincial effects of applying CNOES could be evaluated. Results More soft and hard ware needed to be invested before the CNOES was used. The time to collect scores was significantly less than that by tradtional written exam method（ P 〈0.01）. Examiners thought that CNOES was a more innovative exam method that brought convenience to the head nurse＇s work, simplified preparation process before exam and reduced manpower input. Compared with the traditional exam method, CNOES had the advantage of being more targeted,more easy to inquire and keep data,and more objective in exam results. And the examinees hold the positive attitudes toward CNOES. Conclusion Application of clinical nursing operation exam system makes the paperless exam into reality. It improves the work effiency, promotes the scientific management, and provides necessary support for modern clinical teaching.
