摘 要:目的探讨多不饱和脂肪酸对梗阻性黄疸肠道屏障保护作用机制。方法成年健康雄性Wistar大鼠,随机分假手术组、对照组和实验组,对照组和实验组制作梗阻性黄疸模型,假手术组和对照组采用标准饲料喂养,实验组除标准饲料外,添加多不饱和脂肪酸喂养。偶氮基质显色鲎试验(LAL)定量测定血浆内毒素含量,苯甲酸钠-咖啡因比色法测定结合胆红素含量,免疫组化SABC法测定小肠粘膜固有层IgA+细胞数量,原位杂交法测定IL-6mRNA细胞数量。结果对照组内毒素及结合胆红素含量较假手术组明显升高(P<0.01,P<0.001),而实验组大鼠两个指标的含量显著低于与对照组(7、14 d分别比较,P<0.01,P<0.05);对照组小肠粘膜固有层IgA+及IL-6mRNA+细胞数量梗阻性黄疸时明显减少(P<0.01),但实验组明显高于对照组(P<0.01,P<0.05)。结论多不饱和脂肪酸可以减轻由梗阻性黄疽引起的肠道机械屏障功能的损害,增加肠粘膜固有层IgA+及IL-6mRNA+细胞数量,降低血浆内毒素含量,对梗阻性黄疽的预后有改善作用。Objective Approach the protection mechanism of PUFA( polyunsaturated fatty acid) on the intestinal barrier of rats with obstructive jaundice. Methods The grown - up health rats, were divided into Sham -operation group, control group and experimental group stochastically. Obstruction jaundice model were made in control group, experimental group. The rats in sham -operation group and experimental group were feed with standard fodder, while PUFA were added in the experimental group. Improved limulus lysate test was used to detect endotoxemia level; benzoate- caffeine colorimetric method was used to determined direct bilirubin contents; Immunohistochemical SABC method determined small intestinal mucous membrane inherent IgA^+~ cell number, hybridization in situ method determined IL -6mRNA cell number. Results The endotoxine and direct bilirubin content of control group were obviously higher than that of sham- operation group(P 〈 0. 01 ,P 〈 0. 001 ), the endotoxine and direct bilirubin content of experimental group were lower than that of control group obviously(Td and 14d compared,P 〈 0.01,P 〈0. 05); The intestinal mucous membrane inherent IgA^+ cell and IL-6mRNA^+ cell number reduced obviously under the obstruction jaundice condition , but experimental group were higher than that of control group obviously(P 〈0. 01, P 〈0. 05). Conclusion PUFA may reduce the intestinal mechanical barrier function harm which causes by the obstruction jaundice, increases the intestinal mucous membrane inherent level IgA^+ cell and IL-6 mRNA^+ cell number, reduces the intestinal bacterium content, has the improvement function to the obstruction Jaundice prognosis.
