甲状腺相关性眼病眶部^99Tc^m-生长抑素类似物受体显像与CAS评分的相关性研究

摘　　要：目的探讨甲状腺相关性眼病患者^99Tc^m-生长抑素类似物眼眶显像与CAS评分的相关性。方法采用临床活动度评分（CAS）标准，对62例TAO患者分别评分并根据评分结果分为活动组与非活动组，12例健康志愿者作为对照组，对所有患者及健康志愿者行^99Tc^m-生长抑素类似物眼眶显像，并计算出其眼眶生长抑素类似物摄取比值（UR）。结果3组眼眶生长抑素类似物UR均数分别为1．50±0．25，1．08±0．42及1．07±0．20；活动组与非活动组CAS分值分别为5．17±0．48及1．58＋0．86；活动组UR值均高于非活动组和正常对照组（P〈0．01）；非活动组UR值与正常对照组相比，差异无显著性（P〉0．05）；全部患者UR值与其CAS评分具有很好的相关性（r=0．872）；活动组与非活动组UR值与其CAS评分也有较好的相关性（r=0．789，0．758）。结论^99Tc^m-生长抑素类似物眼眶显像在诊断TAO是否处于炎症反应活动期具有很高的应用价值；^99Tc^m-生长抑素类似物眼眶显像与CAS评分高度相关，有良好的一致性，且更客观、直观。Objective To investigate the correlation of orbital scintigraphy with 99Tcm-somatostatin analogue and clinical activity score in patients with thyroid associated ophthalmopathy.Methods There were 62 cases of TAO and 11 volunteers without eye disease or.thyroid disease （control group, CG）.Based on the clinical activity score （CAS）,all TAO patients were divided into 2 groups,active group （AG）and inactive group （IAG）.All the patients and volunteers were performeded orbital scintigraphy with ^99Tc^m-somatostatin analogue and calculated the uptake ratios（UR）.Results The mean number of the ^99Tc^m-somatostatin analogue orbit uptake ratio of three groups were 1.50± 0.25,1.08±0.42 and1.07± 0.20 respectively;the clinical activity score of AG and IAG were 5.17± 0.48 and 1.58± 0.86 respectively.UR of AG is higher than that oflAG and CG （P〈0.01）.There was no significant difference between IAG and CG （P〉0.05）.Statistically significant correlation was found between CAS and UP, （URallvs.CASall,r = 0.872;URact vs.CASact,r = 0.789;URinact vs. CASinact,r =0.758）.Conclusions orbital scintigraphy with ^99Tc^m-somatostatin analogue is a fairly objective method for judging activity of inflammation in TAO based on the distribution of somatostatin analogue receptor.There is a good correlation between CAS and UR.
