白内障超声乳化吸除人工晶状体植入术后盲与低视力研究

摘　　要：目的探讨白内障超声乳化吸除人工晶状体植入术后盲与低视力的原因，进一步提高白内障手术疗效及降低医疗纠纷的风险。方法手术3个月后复查时检测视力和矫正视力，并进行详细、全面地眼科检查，分析总结96例96只眼白内障术后盲与低视力的原因。结果白内障术后盲12例12只眼，一级低视力38例38只眼，二级低视力46例46只眼，导致盲与低视力的原因主要原因，为术前存在的影响视力恢复的眼部疾患，其中术前已经发现72例72只眼，术后才发现8例8只眼，与手术中并发症有关5例5只眼，后发性白内障9例9只眼，其他2例2只眼。结论随着超声乳化手术技术的提高，术后视力恢复主要取决于玻璃体、视网膜视神经病变，对术前存在这些病变的白内障患者进行复明手术仍有意义，但术前告知病情、与患者良好沟通对降低术后纠纷风险十分重要。Objective To explore the major causes of blindness and low vision after phacoemulsification and intraocular lens implantation.Methods The visual acuity and corrected vision of 96 cases （96 eyes）3 months after the operations were recorded, and the main cause of blindness and low vision was analyzed.Results 12 cases（ 12 eyes）got blindness after the operation, 38 cases（38 eyes）had the first grade low vision,46 cases （46 eyes ）had the second grade low vision.In 80 cases （80 eyes ）the major factors of blindness and low vision were eye diseases existed before the operation.72 cases （72 eyes）were diagnosed before the operation, another 8 cases diagnosed after the operation.5 cases（5 eyes）were related to complications.9 cases（9 eyes）were caused by the posterior capsular opacification, and other causes 2 cases （2 eyes）.Conelusion With the technical development of phaeoemulsification, the recovery of acuity after operation depends on the condition of retina, optic nerve and vitreous body.The operation is still meaningful to the cases with eye disease existed before operation, but the negotiation with the patients before operation is necessary and important to lower the risk of conflict.
