抑郁症自杀行为遗传流行病学对照研究

摘　　要：[目的]探讨遗传因素在抑郁症自杀行为中的作用。[方法]应用病例对照研究方法对103例抑郁症自杀行为先征者、103例抑郁症无自杀行为者以及103例非精神病患者的亲属资料进行遗传流行病学研究。[结果]24.27%的先征者的I级亲属有自杀行为,I级亲属发生率为4.56%,显著高于抑郁症无自杀行为者的I级亲属1.92%(﹤0.001)及对照组I级亲属发生率0.264%(﹤0.001),其OR值及95%可信区间分别为2.44(2.01～2.95)、18.07(2.95～23.68);且先征者的I级亲属自杀行为发生率﹥抑郁症无自杀行为者I级亲属﹥先征者的Ⅱ级亲属(1.60%)﹥先征者的Ⅲ级亲属(1.04%)﹥抑郁症无自杀行为者的Ⅱ级亲属(0.612%)﹥抑郁症无自杀行为者的Ⅲ级亲属(0.39%)﹥群体自杀行为发生率。多基因阈值模型估计,抑郁症自杀行为先征者I、Ⅱ、Ⅲ级亲属的遗传度分别为(71.52±11.02)%、(41.92±11.81)%、(31.09±13.29)%,3者加权平均遗传度为(49.38±3.99)%。抑郁症无自杀行为者Ⅰ、Ⅱ、Ⅲ级亲属的遗传度分别为(46.64±13.27)%、(18.57±16.82)%、(2.02±19.01)%,三者加权平均遗传度为(39.88±4.31)%。[结论]遗传因素在决定自杀行为的易患性上起到重要的作用,自杀行为的遗传方式可能是独立于伴发自杀的抑郁症传递的。[Objective] To explore the effect of genetic factor on the suicidal behaviors （SB） of patients with depressive disorder. [Methods] A case-control study among 103 major depression （MD） with SB, 103 MD without SB and 103 controls was performed to analyze the effect of genetic factor on the liability ofSB. [Results] 25 （24.27%） of 103 probands had one or more first-degree relatives suffered from SB.Prevalenee rate of SB in first--degree relatives in proband group was 4,56%, which was significantly higher than that in MD without SB （1.92%） （P 〈 0.001 ） and controls （0.264%） （P 〈 0.001 ） .Furthermore, the odd ratio （OR） and its 95% confidence interval （95%CI were 2.44 （2.01-2.95） and 18.07 （2.95-23.68）, respeetivelyl The order of prevalence rates of SB was as follows： proband group first-degree relatives 〉 MD without SB first-degree relatives 〉 proband group second-degree relatives 〉 third-degree relatives 〉 MD without SB second-degree relatives 〉 MD without SB third-degree relatives 〉 control relatives. Heritability of SB was （71.52 ± 11.02） % in the first--degree relatives, （41.92 ± 11.81） % in the second--degree relatives, and （31.09 ±13.29） % in the third-degree relatives. The weighted mean heritability was （49.38 ± 3.99） %. Her/tability of SB in MD without SB was （46.64 ± 13.27） % in the first-degree relatives, （18.57 ±16.82） % in the second--degree relatives, and （2.02 ± 19.01 ） % in the third-degree relatives. The weighted mean heritability was （39.88 ± 4.31） %. [Conclusion] Genetic factor might play an important role in the liability of SB in MD.Genetie mode of SB might be independence in the transmission of the suiside of MD.
