高压氧对脑梗死患者血液流变学及神经功能康复影响的临床研究

摘　　要：[目的]探讨高压氧(Hyperbaric Oxygen HBO)对脑梗死患者血液流变学、神经功能康复影响及临床实用价值。[方法]HBO治疗48例脑梗死患者,观察其血液流变学各项指标治疗前后变化,同时也观察患者神经功能缺损治疗前后的变化及治疗有效率,进行统计学处理、对比分析。[结果]48例脑梗死患者经HBO治疗后,血液流变学指标中全血黏度(高、中、低切变率)、血浆黏度、红细胞压积、红细胞聚集指数、红细胞电泳指数及血小板聚集率(1、3、M)较治疗前明显下降(P﹤0.05～0.01),红细胞变形指数明显增加(P﹤0.05),仅血细胞沉降率、红细胞刚性指数无明显变化(P均﹥0.05),同时神经功能缺损积分平均值较治疗前明显减少(P﹤0.01),治疗总有效率达87.5%。[结论]HBO治疗脑梗死安全易行,费用不高、对改善患者血液流变学及促进其神经功能康复确有疗效,值得临床推广应用。[Objective] To investigate the impact of hyperbaric oxygen on hemorheology and neural function rehabilitation of the patients with cerebral infarction as well as its clinical practical value. [ Methods] 48 patients with cerebral infarction were treated with HBO, their changes of the hemorheology before and after treatment were observed and compared. Also. we also observed and analyzed theirchanges ofneurologic impairment before and after treatment as well as the effective rate of treatment. [Results] Compared with the hemorheological indices before HBO therapy among 48 patients with cerebral infarction. the whole blood viscosity in high, middle and low shear rate, plasma viscosity, henmtocrit （Hct）, erythrocyte aggregation index （EAI）, erythrocyte electrophoretic time （EPT） and platelet aggregation rate （1, 3. M） were significantly decreased （P 〈 0.05-0.01 ）, while erythroeyte deformity index was significantly increased （P 〈 0.05） ; Only the scores of erythroeyte sedi- mentation rate （ESR） and erythrocyte rigidity index did not showed significant difference （P 〉 0.05）. In addition, the average of neurological functional deficit scales was significantly reduced with the total effective treatment rate of 87.5% （P 〈 0.01）. [ Conclusion ] Hyperbaric oxygen therapy is recommended because of its operational safety and the therapeutic effect in treat- ment. Hyperbaric oxygen therapy can obviously improve the hemorheology of cerebral infarction and promote the functional rehabilitation of nerve, which is worth to be wild used in clinical.
