2型糖尿病患者中医辨证分型与其胰岛素抵抗及IL-6、IL-8水平相关性分析

摘　　要：目的:旨在观察对2型糖尿病(T2DM)患者进行中医辨证分型后其不同证型与胰岛素抵抗(IR)及IL-6、IL-8的相关性。方法:本院内分泌科住院T2DM患者101例,按照阴虚热盛、气阴两虚、湿热困脾、血瘀脉络4型进行分组。选取体检正常对照组30例。观察空腹血糖(FBG)、空腹胰岛素(FINS)、IL-6、IL-8水平,以ISI指数评估IR。结果:T2DM各证型均存IR,以气阴两虚、湿热困脾及血瘀脉络组显著高于正常对照组及阴虚热盛组(P<0.05或P<0.01),阴虚热盛组高于正常对照组,但无显著差异(P>0.05)。T2DM各证型IL-6明显高于正常对照组(P<0.05或P<0.01),气阴两虚组明显高于阴虚热盛组(P<0.05)。各证型IL-8同正常对照组比较,无显著差异(P>0.05)。结论:T2DM中医各证型IR有不同变化。IL-6可作为T2DM中医辨证分型的参考指数。Objective: To observe the relevance of the insulin resistance(IR) and interleukin-6(IL6) and interleukin-8(IL8) of different types of type 2 diabetes mellitus patients(T2DM) after TCM differentiation of syndromes.Methods: 101 patients were classified into 4 groups,i.e.a group of intense heat from yin deficiency,a group of deficiency of both qi,a group of spleen besieged by damp-heat and yin and a group of blood stasis meridians.Another 30 patients were selected as a control group.FBG,FINS,IL-6,IL-8 were observed to take ISI as an index to evaluate IR.Results: All Types of T2DM had IR,among which IR of the spleen besieged by damp-heat group,the deficiency of both qi and yin group and the blood stasis venation group was higher than that of the intense heat from yin deficiency group and the control group(P<0.05or P<0.01),but IR of the intense heat from yin deficiency group was higher than of the control group and showed no significance difference(P>0.05).IL-6 of all types of T2DM was obviously higher than that of the control group(P>0.05or P<0.01).IL-6 of the deficiency of both qi and yin group was obviously higher than that of the intense heat from yin deficiency group(P<0.01).IL-8 of all types of T2DM showed no significance difference(P<0.05),comparing with the control group.Conclusion: IR of all types of T2DM has different changes and IL-6 can be taken as a reference index to differentiate T2DM types of TCM.
