摘 要:目的:较系统地了解原发性肝癌患者的舌下络脉变化特征,并从舌尖微循环的角度探讨其形成的机理并提供初步的实验依据。方法:从舌局部微循环、舌体局部血管和门脉系血管形态学和血流动力学的角度探讨肝癌异常舌下络脉的形成机理。结果:肝癌患者的舌尖微循环与22例健康人比较,表现为乳头数目增多(P<0.05),在微血管数和血管扩张上均存在显著性差异(P<0.05),在血管扩张上2组比较有明显差异(P<0.01),在微循环总分上,2组比较有明显差异(P<0.01)。不同舌下络脉分级的舌尖微循环各指标差别较大,从微循环单位面积舌乳头总数看,各级比较无明显差异(P>0.05),主要表现为菌状乳头数数目的增多;菌状乳头厚度减小(P<0.05);从微血管数目上看各级比较无明显差异(P>0.05),但在血管扩张上,3级与0级存在显著性差异(P<0.01);在微循环总分比较上,3级与0级有明显差异(P<0.01)。结论:原发性肝癌患者异常舌下络脉的形成,除了与舌局部循环异常有着直接联系外,还可能与全身血管系统及血流动力学和流变学等的异常密切相关,值得进一步研究。Objective: To explore the mechanism of the tongue microcirculation of primary liver cancer patients(PLC) and provide an experimental basis for the future clinical study.Methods: WX-9 microcirculation microscope was used to observe the formation of abnormal lingual venation of 80 PLC patients in terms of local tongue microcirculation,tongue blood vessel,blood vessel morphology and hemodynamics.Results: Comparing with 22 healthy persons,the tongue microcirculation of the patients showed the increase in the number of papillae(P<0.05) and a significance difference(P<0.05) in the number of micro blood vessel and anapetia and a significance difference(P<0.01) in the dilation of blood vessels.In the total score of the microcirculation,the comparison of the two groups showed obvious difference(P<0.01).Each index of the tongue microcirculation of different sublingual venations showed a great difference.From the microcirculation area and the total number of papillae,different levels showed no significance(P>0.05),in which the number of mycelioid papillae increased while the thickness of the mycelioid papillae reduced(P<0.05).From the number of the micro blood vessels,the comparison of each level showed no significance difference(P>0.05),but in the dilation of the blood vessels,3-level and 0-level showed a significance difference(P<0.01) and in the total score of the microcirculation,3-level and 0-level showed an obvious difference(P<0.01).Conclusion: The formation of the abnormal lingual venation of the primary liver cancer patients may have a close relation with the abnormality of vascular system and blood flow dynamics and rheology besides the direct relation with the abnormality of the local tongue microcirculation.
