西藏地区角膜病200例临床分析

摘　　要：目的:探讨西藏地区的角膜病发病情况及特点。方法:收集2006-08/2008-08我院治疗的各种角膜病患者共200例进行整理和分析。结果:发现感染性角膜病主要以细菌性角膜炎最多、占59.0%,其次单疱病毒性角膜炎占34.5%、角膜变性4.0%、真菌性角膜溃疡1.5%、角膜化学伤和角膜热烫伤仅占1.0%。细菌性角膜炎农村高于城市,两者比较有显著差(P<0.05)。单疱病毒性角膜炎汉族明显多于藏族,两者有非常显著性差异(P<0.01),和以往的单疱病毒性角膜炎和种族无关的观点有所不同。真菌性角膜溃疡3例占1.5%,而在过去10a里尚未见过。结论:西藏感染性角膜炎以细菌性角膜炎最多,这和当地的医疗条件落后和健康意识淡薄有关。单纯疱疹性角膜炎藏族发病率明显低于汉族,这和以往的观点不同,有待进一步的基因学研究。而真菌性角膜溃疡患病率有逐渐上升趋势,值得引起重视。AIM： To explore the general conditions and features of corneal diseases in Tibet Autonomous Region. METHODS; We collected and analyze 200 patients with infectious corneal diseases, who were in the out-patient department of the Peoples＇ Hospital of Tibet Autonomous Region diagnosed and treated from August 2006 to August 2008. RESULTS： The facts showed that cases with bacterial keratitis constituted the majorities of infectious corneal diseases which took up 59.0%, the second was herpes simplex keratitis which took up 34. 5%, the rest were corneal degeneration with 4. 0%, fungal keratitis with 1.5% and corneal chemical injury and corneal heat burn with 1.0%. Moreover, bacterial corneal diseases founded in rural areas were more than that of the cities, and there had a notable discrepancy when compared （ P 〈 0. 05）. And different from the view that HSK were not founded more in a specific ethnic group than other ones, we could see that the victims of HSK in the Han ethnic group were more than that of Tibetan ethnic group, and there had a notable discrepancy when compared （ P 〈 0. 01 ）, and fungal corneal keratitis were rare and not founded in the past 10 years. CONCLUSION： The incidence of bacterial keratitis is common in Tibet because of limited and low level medical services for people together with senses of healthcare which lag behind. Besides, the fact that the victims of HSK in the Han ethnic group is more than that of the Tibetan ethnic group is worthy of genetic research, moreover, the trend of rising incidence of fungal corneal ulcers is worthy of attention.
