胃肠道癌组织中CK7、CK20的表达及临床意义

摘　　要：目的:研究CK7和CK20在胃肠道癌组织中表达状况及临床意义。方法:应用免疫组织化学EnVision法研究126例胃肠道癌及癌旁黏膜组织CK7和CK20的表达情况。结果:67例胃癌组织中CK7和CK20的表达分别为88.06%和73.13%,癌旁正常黏膜组织中CK7和CK20的表达分别为16.42%和13.43%(P<0.05)。59例大肠癌组织中CK7和CK20的表达分别为64.41%和96.61%,癌旁正常黏膜组织中CK7和CK20的表达分别为10.17%和15.25%(P<0.05)。低分化胃肠腺癌中CK7和CK20的表达明显强于高分化腺癌(P<0.05)。结论:胃肠道原发癌组织中可表达CK7和CK20,其强表达与胃肠癌的分化程度密切相关,CK7和CK20可作为胃肠道癌不良预后的一个标志物。Objective To study thc expression of CK7 and CK20 in gastrointestinal adenocarcinoma and clinical significance. Methods The expression of CK7 and CK20 in tumor tissues and in adjacent mucosa from 126 patients with gastrointestinal adenocarcinoma were examined immunohistochemically by the EnVision mcthod. Results The positive rates of CK7 and CK20 in expression in the gastric adenocareinoma tissues from 67 patients were 88.06% （59/67） 73.13（49/59）, respectively. The positive rates of CK7 and CK20 in expression in the adjacent mucosa were 16.42% （ 11/67） and 13.43 （9/59）, respectively; there was significant difference between the two groups （ P 〈 0.05 ）. The positive rates of CK7 and CK20 in expression in the eoloreetal adenocarcinoma tissues from 59 patients were 64.41 （38/59） and 96.61% （57/59）, respectively. The positive rates of CK7 and CK20 in expression in the adjacent mucosa were 10. 17% （6/67） and 15.25（9/25）, respectively（ P 〈 0.05） ; In gastrointestinal adenocareinoma, the positive expression of CK7 and CK20 in poorly differentiated grade group was higher than that in well differentiated grade group（ P 〈 0.05）. Conclusion There are CK7 and CK20 expressions in the cases of primary gastrointestinal adenocarcinoma, and the over expression are closely associated with differentiated gastrointestinal adenocarcinoma. CK7 and CK20 may be a poor prog- nostie biomarker of the gastrointestinal adenocarcinoma.
