摘 要:目的通过观察益气回阳注射液对心脏骤停大鼠心肺复苏(CPR)后血清中丙二醛(MDA)、超氧化物岐化酶(SOD)的影响及血白细胞、心肌细胞中细胞黏附分子-1(ICAM-1)表达的影响,探讨益气回阳注射液在CPR后缺血再损伤防治中的可能分子机制。方法将50只清洁级雄性Wistar大鼠采用SAS 6.12数字随机分为5组:益气回阳组、益气回阳+肾上腺素组(简称中西药合用组)、肾上腺素组、模型组、正常对照组,每组10只。采用夹闭气管窒息法制作心脏骤停大鼠模型,在自主循环恢复30min后处死动物,应用流式细胞技术,测定血清中MDA、SOD及血白细胞、心肌组织中ICAM-1的表达。结果模型组大鼠血清MDA含量较正常对照组明显升高(P<0.01),益气回阳组、肾上腺组、中西药合用组血清MDA的含量较模型组显著下降(P<0.01),中西药合用组与益气回阳组和肾上腺组比较差异有显著性(P<0.05);模型组大鼠血清SOD含量较正常对照组明显降低(P<0.01),益气回阳组、肾上腺素组、中西药合用组SOD含量较模型组显著升高(P<0.01);中西药合用组与益气回阳组和肾上腺素组比较差异有显著性(P<0.05);大鼠模型组血白细胞及心肌ICAM-1的表达较正常对照组升高(P<0.05),而益气回阳组、肾上腺素组、益气回阳+肾上腺素组ICAM-1的表达较模型组显著下降(P<0.01),三者组间比较无显著差异(P>0.05)。结论益气回阳注射液能减少心脏骤停后血清中MDA的生成,增加SOD活性,对心肺复苏后再灌注损伤有一定的治疗作用;益气回阳注射液可能通过抑制血白细胞及心肌ICAM-1表达,在一定程度上减轻复苏引起的心肌细胞的损伤。Objective To investigate the influence of Yiqihuiyang injection On the levels of MDA and SOD in rats after cardiopulmonary resuscitation(CPR) cardiac arrest rats, and its possible molecular mechanism. Methods 50 clean male rats were randomly divided into five groups, Yiqihuiyang group, combined Chinese and western medicine group, epinephrin group, model group, and normal control group ,with 10 rats in each group respectively. The rats were executed and the hearts were isolated 30 min after the recovery of sutonomic circulation. The levels of blood MDA, SOD were measured in each group and the protein expression of ICAM -1 was determined by flow cytometry. Results The content of MDA in model group was significantly higher than that in control group(P 〈 0.01 ), and the activity of SOD in model group was significantly lower than that in control group (P 〈 0.01 ). The concentrations of MDA in epinephrin group, combined Chinese and western medicine group were significantly lower than that in control group, and the activity of SOD in these groups were significantly higher than that in control group ( P 〈 0.01 ). In the three groups the combined chinese and western medicine group had more significant influence on the blood Levels of MDA and SOD. On the other hand, the expression of ICAM - 1 of the model group was different from the control group ( P 〈 0.05 ). The expression of ICAM - 1 was inhibited in epinephrin group, Yiqihuiyang group and combined Chinese and western medicine group( P 〈 0.05 ). The three groups had no significant difference( between them P 〈 0.05 ). Conclusion Yiqihuiyang injection may attenuate reperfusion injury after CPR through reducing MDA and increasing SOD activity. At the same time, Yiqihuiyang injection has an effect in reducing the myocardial injury induced by resuscitation by blocking the expression of ICAM - 1.
