外周血表皮生长因子受体mRNA表达对胃癌微转移和预后的价值

摘　　要：目的:分析检测胃癌和胃良性病患者外周血中表皮生长因子受体(epidermal growth factor receptor EGFR)的表达情况,探讨其对胃癌微转移和预后的价值。方法:应用逆转录聚合酶链反应(RT-PCR)检测57例胃癌,及40例胃良性病变患者外周血EGFRmRNA。结果:40例胃良性病变患者对照组外周血EGFR均为阴性。胃癌患者组EGFR mRNA表达阳性为42.1%(24/57)。EGFRmRNA表达阳性在胃癌TNM分期中为I期27.3%(3/11),II期15.8%(3/19),III期61.9%(13/21)及IV期83.3%(5/6),(p<0.05)。EGFRmRNA表达阳性在低未分化及中高分化癌间有显著差异(p<0.05)。结论:胃癌患者中外周血中EGFRmRNA可以作为检测胃癌患者外周血微转移的分子标志,可作为胃癌微转移及预后的参考指标。Objective：To explore the epidermal growth factor receptor（EGFR） in peripheral blood of the patients with gastric cancer and benign gastric diseases, and evaluate its clinical value for diagnosis of micrometastasis and prognosis. Methods：The expression of EGFR mRNA in peripheral blood was examined by reverse transcription polymerase chain reaction（ RT- PCR）in 57 patients with gastric cancer and 40 benign gastric diseases. Results： In peripheral blood from 40 benign gastric diseases, the EGFR mRNA was not expressed. The expression of EGFR mRNA in patients was gastric cancer42.1% （24/57）. The expression of EGFR mRNA in TNM clinical stage was found in I 27.3 %（3/11）, Ⅱ 15.8%（3/19）, Ⅲ61.9%（13/21） and Ⅳ83.3%（ 5/6 ）, （ p 〈 0.05 ）. There were differences of the EGFR expression rate between the patients with poor differentiated and high and middle differentiated cancer（ p 〈 0. 05 ）.Conclusion ：The expression of EGFR mRNA in peripheral blood of patients with gastric cancer can be used as prospective biomarkers for detecting micrometastasis and prognosis.
