摘 要:目的探讨不同部位静脉注射阿霉素对处于亚临床阶段兔心脏血流动力学的影响。方法取实验兔12只,从耳缘静脉推注阿霉素2.0 ml/kg,每周1次,共4周,制作实验兔心脏亚临床型毒性模型。兔末次给药后12周通过颈总动脉心室内插管,记录麻醉状态的血流动力学指标结果,左心室压力波形稳定40 min后,随机分为耳缘静脉组及中心静脉组各6只,分别在耳缘静脉注射和经颈外静脉插管到上腔静脉注射阿霉素2.0 ml/kg,给药后30 min比较两组血流动力学指标结果。结果两个部位用药后30 min亚临床阶段兔心脏血流动力学指标结果比较,差异均有统计学意义,中心静脉组左室收缩压、左室舒张末期压较耳缘静脉组下降(P<0.01);左室最大收缩速率和左室最大舒张速率减弱(P<0.01或P<0.05)。结论从回心路径短的中心静脉给药会使处于亚临床阶段的兔心脏收缩功能下降。建议临床使用阿霉素时尽量选择离心较远的上肢静脉给药。Objective To explore the influence of adriamycin intravenous(Ⅳ) at different locations on Rabbits' Cardiac Hemodynamics. Methods 12 rabbits for experiment use were made sub-clinical heart toxicity models by means of IV injection at the ear vein with 2.0 ml/kg of adriamycin, 1 time per week for 4 weeks. 12 weeks after the final injection, the ventricle was incubated through the carotid artery. Then the hemodynamic parameters of the rabbits under anaesthesia were recorded. 40 mins after the pressure waveform of the left ventricle stablized, the 12 rabbits were randomly divided into two groups, ear vein group and central vena group, each with 6, with one group injected at the ear vein, the other injected by means of intubation through external jugular vein into superior vena cava, of ardiamycin, 2.0 ml/kg. 30 mins after the injection, a comparison was made of the hemodynamic parameters between the groups. Results The hemodynamic parameters of the two groups demonstrated a statistically significant difference, with the end-systolic pressure and end--diastolic pressure of the left ventricle of the central vena group lower than that of the ear vein group (P〈0.01), and the maximum systolic and diastolic rates of the left ventricle were lowered (P〈0.01 or P〈0.05). Conclusion Adminstration of drug at the central vena, which is nearer to the heart, will reduce the cardia systolic function of sub-clinical rabbits. It is recommended that administration of adriamycin be performed through the upper limbs vein, which is farther from the heart.
