摘 要:目的观察给予促红细胞生成素(EPO)对脑创伤(TBI)患者外周血中脂质过氧化物酶(LPO)、内源性过氧化物歧化酶(SOD)的影响。方法将70例TBI患者随机分成2组,即治疗组和病理对照组,入院后均进行CT检查、GCS评分,根据GCS评分将各组又分成轻、中、重3组。所有患者均按照常规进行治疗,治疗组患者在常规治疗基础上加用皮下注射EPO,并于伤后24h、治疗3周后即刻及治疗后1个月、3个月抽取外周血进行SOD及LPO水平的检测,于治疗后1个月进行格拉斯格预后评分(GOS)。选择来院体检的正常人群为对照组。结果给予EPO治疗后的中度TBI患者在治疗后即刻、1个月及3个月外周血中SOD及LPO水平与病理对照组比较差异均有统计学意义(P<0.05),治疗后1个月的GOS评分与病理对照组比较亦差异显著(P<0.05);轻度及重度患者在上述时间点上SOD、LPO水平及治疗后1个月的GOS评分与病理对照组比较差异不明显。入院初病情等级与SOD/LPO比值呈负相关,而治疗组治疗后1个月GOS评分与SOD/LPO比值呈正相关。结论TBI后SOD/LPO平衡受到破坏,及时给予EPO治疗,可以恢复SOD/LPO平衡,改善预后。Objective To investigate the effect of Erythropoietin (EPO) on the lipid peroxydase (LPO) and superoxide dismutase (SOD) in peripheral blood of traumatic brain injury (TBI)patients. Methods Dividing all patients into two groups randomly:treatment group and patho-control group. CT scanning and GCS score were all performed. The patients in such two groups were luther divided into gentle, moderate and serious subgroups according to the GCS scores. The patients in treatment group were added EPO subcutaneous injection except for the rule cure. The levels of SOD and LPO of all patients were detected 24 hours after TBI and 0 day,1 month ,3 months after treatment. The GOS score was used 1 month aftre treatment to speculate the prognosis. Patients of the normal control group were recruited fi'om health examination people. Results The level of SOD and LPO of patients in moderate-TBI treatment group showed significant difference compared with patho-control group at 0 day, 1 month and 3 months after treatment ( all P 〈 0. 05 ), and the GOS score increased compared with patho-control group at 1 month after treatment (P 〈 0. 05 ). However, the levels of SOD and LPO at above time point and the GOS score at I month after treatment in gentle and serious group showed no significant differences with patho-contrul group ( all P 〉 0. 05 ). The ratio of SOD/LPO has inverse correlation with state of TBI at in-hospital and positive correlation with the GOS score at 1 month after treatment. Conclusions The balance of SOD/LPO was destroyed after TB1. EPO supplement could assist therapy and ameliorate the prognosis, and the mechanism might recover the balance of SOD/LPO.
